Suggest Treatment For Slightly High Cholesterol And Vitamin D Defiency

Question: Hi, I am 32 years male. 6 feet tall and 87 kgs of weight. Both parents have hypertension. I recently went for my annual check up and was diagnosed with slightly high cholestrol and Vitamin D deficiency (reports uploaded). Was diagnosed with fatty liver 6 months ago but now okay after dietry control. I have also been having slightly elevated BP every now and then in last 1.5 years (at XXXXXXX it has been 140/90 ). While analysing the check up reports, my family physician has suggested me to start dosage for Vitamin D and BP. For BP, i have been suggested Telpres 20 mg. My query is whether i should start medication for hypertension at this age as high BP might be due to stress and anxiety as i am always concerned or thinking about well being of my family and am also in a high pressure corporate job with no time for physical execrcise. I have heard that the BP treatment tablets have side effect and starting them at this age may not be approporiate. So please suggest and also if i start this, would it have any effect on my sexual life (am newly married and yet to start a family). Also, can i wait for 6 months, make lifestyle changes, have dietry control, monitor my BP regularly and then start medicine, if it still remains elevated. Please suggest. And also if i have to start medicine is Telpres (ARB) okay for young patients or i should be cosnulting other doctor for alternate category such as Beta blockers. Thanks.

You need to take vitamin D – large doses for 4 to 6 weeks and then maintenance dose& follow up

There are subdivisions of cholesterol – LDL, HDL, non-HDL and so on.
HDL is good cholesterol – higher, the better.
LDL is bad cholesterol – lower, the better. .. Your level is high (130) / Aim for 70 because of family history of pressure and your own borderline.
In the absence of any other risk factor 100 is ideal for LDL-C. In the presence of ischemia, 70 should be the aim.

     Keep your weight to normal / even a little less
     Quit smoking, if you are a smoker
     Do regular EXERCISE – say walking or jogging - 30 minutes a day / at least 5 days in a week. Isometric exercise (like bull worker) is bad.
     Healthy FOOD is a must. Avoid saturated fats – coconut oil, palm oil, butter, ghee, full cream milk...Avoid junk foods in particular – French fries have palm oil / Pizza has cheese / XXXXXXX sweets are made in ghee.../ Change to Poly-un-saturated oil – like sun flower oil. / Include Mono-un-saturated oil – Olive oil.
Repeat the test in 6 weeks.
     Cholesterol can still be high, in spite of diet and exercise – you need drugs; because it is also manufactured in the liver – and that is determined by heredity. Statin is the first choice. Some start it straight away – even with normal levels, even without problems (Primary Prophylaxis)

Lab report shows urinary infection – apparently asymptomatic.
Repeat it - a clean void midstream sample / have urine culture and ultrasound / and get treated depending on the result

Thank you doctor for the detailed answer. I also wanted to know that for last 4-5 weeks , i have been getting noise/popping sound from all body joints (shoulder, arm, wrist, knee, ankle) on normal movemnet. family physician said that it might be due to Vitamin D deficiency and has put me on 60K tablets weekly and daily dose of calcium (1000 mg). Could you please confirm if vitamin D deficiency be the actual cause of these popping joints/sound (without pain).

D deficiency generally does not cause pain.
It is preferable to consult a Rheumatologist / have a few relevant investigations to exclude other possible causes
