Suggest Treatment For Shortness Of Breath And Chest Pain

Question: Hello doctor

Lately I have been waking up with shortness of breath in the morning and chest pain. I think I have GERD because I went to ER a few weeks ago and they told me I had symptoms of it. Ever since I had a lump in my throat and stomach pain but now the shortness of breath is a new thing. I also had a history of anxiety and I think it's back I never had any heart problems either. What can it be? Thank you for your help

Hello thanks for asking from HCM

You have symptoms of GERD (Gastro Esophageal Reflux Disease) and just recently developed symptoms like shortness of breath and chest pain. You have history of anxiety.

GERD itself can present with epigastric distress and chest pain but symptoms usually occur after meals or in evening hours. You have symptoms more common in morning so the possibility that chest pains and shortness of breath are due to GERD is less.

You never had heart problems but still I would advise you to get your EKG done in order to rule out any heart problem. If EKG comes normal then we are left with anxiety as possibility of such symptoms.

Anxiety can present with chest pains, breathlessness, multiple somatic complaints like symptoms. Consult a psychiatrist for proper evaluation and treatment.

In anxiety associated with pain conditions and somatic complaints medicines like SNRIs especially Duloxetine, Venlafaxine etc prove useful. For symptomatic treatment medicines like Propranolol can provide relief. These medicines will reduce the intensity of chest pains and will provide relief in breathlessness. Visit a psychiatrist for prescription.

I would also advise you to try relaxation exercises and yoga especially "Pranayama"(a type of breathing exercise) in morning. This will help to reduce the symptoms of chest pains and breathlessness. Progressive muscle relaxation exercises will help to reduce the symptoms of anxiety in day time.

As EKG came normal and it was normal so it also points to Anxiety as possibility of your symptoms. Your symptoms are occurring more during day time, start since morning which is also a feature of non-specific anxiety.

So please visit a psychiatrist for help, I would also advise you to follow few life style measures which will help to reduce such symptoms-

~ Exercise daily as exercise helps to secrete endorphins and these chemicals lifts the mood and will help to reduce symptoms of restlessness in morning.

~ Start taking OMEGA-3-FATTY ACID supplements. These supplements helps to reduce anxiety.

~ Take diets rich in proteins and carbs and low in fats. This will help in improving mood. You can also include omega fatty acid diets like Flex Seeds, Walnuts, XXXXXXX nuts, Salmon fish etc. This will also help to reduce anxiety.

~ Sleep for at least 7 hours in night. Good sleep helps to improve the symptoms of anxiety.

~ Try relaxation exercises, progressive muscle relaxation exercises and breathing exercises as I have mentioned in my last answer, this will help to reduce anxiety.

Hello Doctor,

for about a month also I have had stomach problems which come and go. Some days I am hungry and some days not. My stool is smooth in the morning then hard in the afternoon. Sometimes I get full faster, but I have learned to live with it. Is this stomach cancer? I saw the symptoms and they do match in a way, expect I do not have blood in my stool or vomit. I am getting a endoscopy done XXXXXXX 13.

No, this is not stomach cancer, no risk at all

Hello again, thanks for asking a follow up question

First of all, I would like to tell you that its not stomach cancer. You have symptom of abdominal pain but other symptoms of stomach cancer are not there. Altered bowel habits like smooth stools, then hard stools along with symptoms of anxiety when all investigations came normal could point more towards anxiety as reason of such symptoms. Irritable bowel may be one possibility for such symptoms.

You don't have blood in stool or vomitus so we can easily rule out stomach cancer. Secondly, all of your other investigations came normal so the chances of stomach cancer are very very rare. Bowel complaints are not seen in stomach cancer. You never had significant weight loss (not mentioned in question) so we can rule out stomach cancer easily.

Yes, you can go for endoscopy but that investigation will also come in normal range. As per my opinion don't waste your time in worrying, you are perfectly normal except you have anxiety which would improve with proper consultation of a psychiatrist.
