Suggest Treatment For Severe Fatigue In A Person With Hypothyroidism

Question: Good morning Sir, I am a 53 yr w/f. I am having episodes of debilitating fatigue to the point that I cannot even carry on my normal activities. My labs are normal except for hypothyroidism which I take 60mg Armour Thyroid. My labs indicate I'm post menopausal with lowT levels. I have constant dry mouth,inflamed salivary ducts and swollen lymph nodes in my neck and I found one behind my left patella. I've had labs to r/o Sjogren's, IgG, SS-B,SS-A. All were negative. A CT with contrast of head and neck was negative/normal.
I have 2 lumps I can feel and my ENT can feel along my inner lower jaw. He thinks may be duct stones. I'm thirsty all the time but bs is normal. I have severe transient joint pain, wt. gain and hair loss. This has been going on for well over a year but I did not c/o before because I'm an RN and have retired after a 30 yr career working in a State Prison and heard my share of whiners. I never wanted to be one so I just brushed it off but now the fatigue and pain is robbing me of days spent in bed. My brother has RA. My sleep is a mess but I attribute that to working 15 yrs of nightshift and many 16 hr mandatory shifts. I am on Adderall 30mg, Nexium,Estradiol (X last 4weeks as a trial to see if helps),Topamax for migraine prophylaxis, Wellbutrin,Prozac,Celebrex as well as the prior mentioned Armour Thyroid.
I will have severe episodes then I will have 4-5 days of energy. Then it starts all over again. The aches, fevers, fatigue and at times insomnia. Having worked in a prison I asked the physician to have an HIV and Hepatitis panel added every time I had lab drawn. I don't understand how I can feel good then drop off so quickly that I'm too weak to get out of bed. The smallest of task is monumental.

I have gone through your question.
I can understand your concern.

You have a complex past medical history and your actual symptoms are the result of your diseases.(Anyway i see that you have done all the tests and take correctly the treatement for each problem)

Fatigue,weight gain and hair loss and insomnia can be symptoms of hypothyroidism.Hypothyroidism (underactive thyroid) is a condition in which your thyroid gland doesn't produce enough of certain important hormones.(T3 and T4).
For this reason i suggets to repeat the thyroid function tests(TSH, freeT3 and free T4 ) to understand your actual thyroid function because in my opinion you need dose adjustment of the thyroid hormone replacement therapy.(Armour)

At the other side i think that your past has produced a lot of problems in your actual psychological life ,so my advice is to consult a psychologist discuss with him all your stress.I think he/she can help you to feel better.
