Question: Quite daily about 3-4pm, diarrhoea feeling and need to rush to toilet. There's plenty of noise in the stomach..plenty of gas and movements..mostly end up with soft stools. After release, sometimes followed by stomach cramps.

I am sorry for the health problems you are going through.

To summarize them:
- diarrhea feeling daily about 3-4 pm
- noise in the stomach, gas and movements ending up with soft stools and followed by stomach cramps
- medications tried: charcoal, meteospasmyl
- patient: male 49 years old

Now, the presence of diarrhea and increased stomach gas and movements followed by abdominal cramps are suggestive a possible disturbance of your gastrointestinal tract.

As the symptoms occur around 3-4 pm, there are 2 main explanations:
1. You did not consume your lunch, and the noise your stomach is making is due to hyperacidity of the empty stomach.
2. If you have your lunch regularly, then, it could be due to food intolerance. If you are also working and stressed out, that can increase chances for changes in bowel movements.

I advise to make some diet changes first:
- avoid fast foods or chilly/spicy/oily foods that could aggravate stomach hyperacidity
- avoid using straws when drinking fluids
- have your meals regularly, in time
- chew up slowly, do not delay
- take antacids after meals
- simethicone can also help getting gas relief
- take probiotics to stabilize intestinal flora
- try to control stress
- avoid foods/drinks containing caffeine or chocolate

If after such changes, you will not notice any improvement, then, I advise to consult with a gastroenterologist and get further examinations in order to determine the right diagnosis and decide for best treatment strategy.

Abdominal ultrasound, and stool examination and culture testing are advised to run further.
