Suggest Treatment For Severe Constipation While On Treatment For Depression

Question: My mom is psychiatrist patient since last 10 years.
She is currently taking following medication on consultation of psychiatrist since one and half year :-

ZAXPAM 10,
DOXIN 75,
DEPRAN 10,
TRIJEE -H,
REJUVATE OD,
CIPCAL 500.

All tablets dosage is one daily.


As per her mental condition she is alright but she has problem of constipation since she started the medication of depression, i.e. almost 9 years.

Kindly suggest some ways to get rid of constipation

I read your query and understand your concerns.

Among the prescribed medicines Trizee H is most important in contributing toward constipation. Although it will be appropriate to remove the complete Trizee H considering the long duration of action but if it is not possible at least the H part (trihexphenydil) should be removed from the prescription.

Among the women with less mobility the trihexphenidyl causes constipation as side effects and it is possible that with withdrawal of trihexphenidyl can help her tremendously in respect of constipation.

In addition to this three more things are important to ensure proper bowler movements

1. First adequate fluid intake
2. Regular physical exercise
3. Intake of dietary fibres- Ask her to take whole grain foods and fruits which have high finer content.

sir,
she is taking these medications since one and half year back, before that her mental condition was very pathetic, she has tried different medications like libotryp from mar 2007 to may 2013
Amixide and escitalopram from may13 to nov13
amitriptyline from nov13 to may15

and after that she is on the medication as mentioned previously with much better results.

All these 9 years she have very poor bowel movement and complain of constipation.

Her diet includes of carrot, radish, soaked beans, fruits in breakfast
boiled rice, boiled vegetables in lunch
whole grain chapati and vegetables in dinner.

she do yoga related to leg lifting and keen movement for ortho related problems.

she also take sometime kayamchurn but all these are of no significant improvement.

Tryhexphenydil can be removed without fear of relapse

Thanks for reverting back to me.

From the available medication details it is clear that at least one medicine was part of prescription which causes constipation. Initially it was Libotryp and than it was amixide.

In current prescription the trihexphenydil part of TRIZEE-H is not part of prescription but it is there to prevent some side effects only. The side effects include tremor, dystonia, mask like face, decreased movements. If it is possible it should be removed. In case she has developed any of the extrapyramidal symptoms by today it is possible that she may not need trihexphenydil which is contributing toward constipation.

In case you are not willing to remove trihexphenydil (I understand there are fears) the lactulose can be started to improve her bowel movements after discussing with her psychiatrist.
