Question: I have red bumps in a small cluster on my penile shaft, they are not itchy or painful and they are shiny

It seems to be a sexually transmitted disease related genital lesion. You need to get checked for Chlamydia and Gonorrhea from a laboratory.

As a prophylaxis and to treat in the right directions you need to start with antibiotics orally. If I was your treating physician I would have prescribed you with Azithromycin 1g once daily. Along with that apply Hydrozole cream topically. Avoid cream going into the penile opening.

There is a latent period between exposure and the appearance of the genital lesions and sometimes people remain asymptomatic altogether. Infection may even spread through kissing and licking as an asymptomatic individual may also be a reservoir of the infection and oral, vaginal and anal secretions may all transmit the infection.

I have started you with prophylactic treatment as there is no improvement with topical antibiotic and anti fungal. And I have added a steroid cream to cover the infection and inflammation and to cure the redness of the lesions. I would suggest starting it after discussing with your treating doctor.
