Brief Answer:
It could be Hemorrhoids.

Detailed Answer:
Hi.
Thanks for your question.
Hemorrhoids can cause such symptoms.
And you are right about the rectal pressure also. If you strain too much the sphincter pressure can lead to Hemorrhoids. I suggest you start taking mild laxatives like Senna or Isabgol at night to ease your stools and the pressure along with it.
You should be fine with that.
Take care and do let me know if there are any questions on this issue.

Thanks for the quick response is it common for hemmroids to feel like you constantly need to have a bowel movement

Just got back from my GP she performed a rectal exam and thinks the pain is not from hemmroids but from an anal fissure the pain I'm feeling is due to muscle spasms my question is is it normal for anal fissures not to bleed and is heat beneficial for muscle spasms

Brief Answer:
Constipation needs to be treated for these problems to resolve.

Detailed Answer:
Hi,
Anal fissures and Hemorrhoids behave similarly and also are treated similarly. The reason behind both is raised rectal tone and increased sphincter pressure complicated with constipation. If we can take rectal pressure and constipation out of the picture then these problems will not occur.
I suggest you start taking laxatives as I suggested and apply Lignocaine ointment in that area before and after you go for stools and after a little while apply GTN ointment which can help the sphincter to relax. Anal fissures can present with no bleeding. Heat application definitely helps and we advice it in the form of Sitz baths.
If all this does not help then you will need surgery to treat the Fissure which is a minor surgery. But after that surgery also you will need to keep the stools soft for the wound to heal.
Take care,
