Suggest Treatment For Prolonged Productive Cough

Question: I cough for over a month, wet cough, I have a lot of mucus coming from my nose that is translucent and yellow from my lungs. Lately I have pain in my left shoulder blade. I took a lot of over the counter medication for mucus, did rinsing of my nose with budezonide and salt, nasal sprays, allergy medication. The cough is not as persistent, but there is a lot o mucus and when I cough, I cannot always eliminate it from my lungs. SHould I take antibiotics?
Any advice would help. Thank you!

I've gone through your query and understand your concern..

Now it seems like quite prolonged cough , and your symptoms are suggestive of mixed(Upper & Lower) Respiratory Tract infection, so further investigations as well as antibiotic therapy is a must..

Not all cough and cold are allergic, so anti-allergic medications have no role in it..

First of all, you need to get thoroughly investigated including Complete Blood Count, ESR, Chest X-ray(PA view) along with Sputum for AFB x 3 samples as well as sputum Gram stain and culture sensitivity..

It's been a month already so please don't delay anymore, otherwise it'll become even more complicated..

After you've given samples for Sputum-AFB , you should start taking Tab AZITHROMYCIN 500 mg Once Daily for the next 10-14 days ..

For cough & secretions, start taking, Syp BROMHEXINE 10 ml Three times a day after food for next 5 days..

You need to check in to your local physician for prescription as all these are prescription medications only..

Cap PANTOPRAZOLE 40 mg Once Daily before breakfast for 10 days to prevent any underlying acid reflux (as they ten to worsen it further)..

Do Steam inhalation and luke-warm Saline water gurgling three to four times a day..

You can also try Saline nebulization as it'll help to thin out the secretions and help you to cough them out.

Stay well hydrated, drink lot of hot tea, chicken soup , porridge etc..

Besides , If I were your treating doctor , I'd ask to for a complete clinical evaluation to look and listen for any other signs and symptoms of any underlying pathology..

Please try to avoid any kind of smoking(if you do)..

Thank you very much for your prompt reply. I do not have fever and never did during the past 4 weeks. I coughed up a lung at night, for a few days this week, but for 2 days this doesn't happen anymore. I do not smoke, drink and I do have a healthy diet and I exercise. I am in good shape and had an Xray done last year for a heavy post nasal drip and the lungs looked fine. I was diagnosed with beginning asthma, but I do not have asthma symptoms of suffocation, or anything like that, just post nasal drip. With the nasal rinsing with buzedonide, it was improved but never gone. Now I have more post nasal drip ( which I know can be accentuated by a cold) and yellow sputum when I cough (from my chest). I do not cough a much, but I still get nasal congestion at night in one nostril. My blood tests last year were fine, but did not do any new ones this year. I'll do that and sputum testing - do I go to an allergist for that or ENT? Also, I have Rovamycine available - 1.5 mil U.I. Should I take it for 8 days or so, even though I have no fever? Please let me know your thoughts. My immunity doesn't seem to great lately and I attribute it to the antibiotics, because I get these sinus infections and respiratory problems every winter and spring for the past 5 years now and I end up on antibiotics. Should I check anything else? After an episode like that I visited the allergist who tested my lungs, blood and everything seemed fine. He did not test my sputum. Sorry for the long email, but I hope this can give you more details to diagnose via email.One more detail, with this cold I also started to have pain in my hips and knee. Am I at risk of developing arthritis ( my mom has it) or any other auto immune disease? Sorry for the stream of emails. Thank you! Thank you very much. XXXXX

Hello Dear, welcome back..

Appreciate all these details as you've rightly mentioned , they're indeed important for a proper clarification as well as diagnosis..

Even though you symptomatically better now , still I'd suggest you follow all those advises as I've already mentioned in my previous reply..

No , I wouldn't recommend you to take Rovamycin(Spiramycin).. In my opinion you should go for Azithromycin after giving sample for the sputum tests..

Now it's been one year already, on top of that with these recurrence , I'd advice you to go for repeat Chest X-Ray in this regard..

Early onset asthma, can get precipitated by allergies, so in your case I'd also suggest you to get a Comprehensive Allergy Panel test to find out the offending agents and get managed accordingly..

You also need to get a PFT(pulmonary function test) with and without bronchodilator, and after all these tests , you need to schedule an appointment with the PULMONOLOGIST (neither Allergist nor ENT) to get yourself thoroughly examined and plan the next course of action as per the findings..

Stop using on-off Steroids, as they tend to worsen your immunity if not taken without a proper indication and proper tapering..

Yes, considering this recurrence, it'd be a good idea to get in touch with the Pulmonologist at this instance..Recurrent Post nasal drip can be there due to a deviated nasal septum or sinusitis, which even the pulmonologist can diagnose, no need to go for ENT additionally..

Meanwhile get yourself investigated and take the medications as already advised, you'd be feeling better..

As for the last part of your query, not all arthritis are having familial pre-disposition, so need to know which type of arthritis do you mean.. however auto-immune disease is a very broad term, and the symptoms' lists are just way too similar to further comment on that, still I'd request you not to get all such diseases in your head, as the more you'll search, with increasing names and similar findings- you'll soon start feeling that you're having so many of them , all at the same time, Confusing isn't it ?

So, the best way is not to think about them too much and stay happy and engaged with your life ..
