Suggest Treatment For Premature Ejaculation

Question: hi, i am 45 years age, male. I am not a diabetic and have no other conditions. I have problem of premature ejaculation and i am unable to last for more than 5 seconds. My penis is also becoming thinner. This is leading to problems with my wife. please help. thanks,

I would like to mention that I am a psychiatrist and problem of premature ejaculation(PME) is generally treated by Psychiatrist , not by diabetologist, that's why I am answering your query.

PME is defined as ejaculation of man occurs before female partner reaches to orgasm during intercourse.
If you are suffering from it and you are non diabetic, there are two types of treatment.

One is medicines: Clomipramine (Dose 10-25 mg), Paroxetine (10-20 mg) and depoxetine(30 mg) are useful.
Clomipramine or paroxetine can be taken initially daily and later on just before 2-3 hour of sex under doctor's guidance.While depoxetine should be taken 2 hours prior to intercourse only, however availability of depoxetine is the issue.
No significant side effects of these drugs. If I were your treating doctor, I would go with 10 mg clomipramine to start with.

Second is sexual counselling: In which couple counseling matters as sex is an act of two partners.In this "stop and squeeze technique" , "Master & XXXXXXX technique" to be learnt from qualified sexologist or psychiatrist.If during counseling issues of performance anxiety found out, they can be solved also.

By saying "penis becoming thinner" do you want to convey erectile dysfunction problem(Difficulty in erection) or is it just the concern and you are facing PME after erection is the only problem?" You can clarify, so I can help you more.

thanks for the response. I will start the medication option -- 10 mg Clomipramine. please let me know when should I take the tablet -- afternoon, night, before / after food, etc.

Also, can you please refer me to a qualified sexologist in XXXXXXX suburbs, who can help me learn the 2 techniques?

also to add, once i ejaculate, it takes me about 12 hours to get an erection again despite all efforts. please help me resolve this problem as well. thanks,

Clomipramine should be taken after half an hour of dinner.

There are many psychiatrists in XXXXXXX whom I know.But I will like to mention one name for your problem. Dr.Rajshekhar Brambhatt, Direcotor, Kama Institute of Sexual Studies & Counselling center-BCJ Hospital XXXXXXX I do not have exact address but I think you find detail address.
Others Dr XXXXXXX Shah, Dr.Ashit Sheth, Dr.Vihang Vahia.

Normally in all men there is "refractory period" after ejaculation during which again erection and ejaculation is not feasible.It ranges from 30 to 90 minutes.
However You are mentioning 12 hours duration. Frankly speaking I can not get that how you can measure that it is taking 12 hours.
But of course your problem is problem. I would suggest checking your blood pressure once, if it is normal you need not worry. In any case you can start one medicine called tetrafol plus (Methyfolate+Mecobylamine+Pyridoxal phosphate), which is combination of vitamins, after meal two times a day. It will help to dilate your penile vessels and so in erection. But still you need to consult doctor for this problem for detail evaluation and examination.
