Suggest Treatment For Painful Ulcers

Question: It doesn't appear that my ulcer is healing on the pantoprazole 40mg twice a day because I'm still in pain. I'm taking Carafe 1 gram before meals and bedtime and ranitidine 300mg am and pm. I followed the diet restrictions and am taking the .meds religiously. I feel because I've been on meds for over 20 years they don't work anymore. Does Medicare pay for Nissan functions? Would that be a option? I had a 24 hr ph monitoring 10 years ago and it showed high acid levels. I also have esophageal spasms and had a motility test which was positive 10 years ago. I feel the spasms are more from irritation rather any structural problem. The radiofrequency ablation didn't help my back and since I can't have steroid injections anymore am at a lost what to do. I'm going to get better about the exercises and loose some weight. Does a tens unit work? I'm taking Tylenol #3 and flexeril sparingly. I this think I'm going to have to increase them if I'm going to have any quality of life. I know doctors are against this but I don't think it's fair to be in constant pain, interrupted sleep and the anxiety it causes. I'm still having diarrhea several times a day. Sometime I can't make it to the bathroom which is very embarrassing. I have to take anti diarrhea pills if I want to leave the house. I have been avoiding going out because of this.

Yes I have reflux. I have been told in the past I have a hiatal hernia I didn't ask 8weeks ago when I had my scope. I have burning pain mostly after I eat and a aching pain. Burning pain in my back. Hurts in throat and chest went I swallow. I also gets spasms with cold liquids and tightness difficulty get food down with bread and meat. My endoscopy said I had a bleeding ulcer in the upper cardiac area. Ulcer in lower stomach and one in the duodenum with a lot of inflammation. I have idiopathic polyneuropathy. The gastric emptying in the past showed gastro paresis now it's showing dumping syndrome which my gi feels is due to the neuropathy. He thinks it's what is causing the diarrhea. I take Imodium before going out. It helps. I do not smoke or drink. I limit coffee to one cup a day. I really need the caffeine in the am. The last tome I got a ph test was 10 years ago. I have been test for h pylori it was negative.

I have read your detailed reply and will try to answer as comprehensively as possible.

You must be knowing that your symptoms are well explained by neuropathy , even non healing ulcers as well..


Did your gastro doctor get your gastrin levels checked? some times gastrinomas cause no healing ulcers.

For better result in gastro therapy your nerve issue have to be dealt with by neurology, i would suggest you to be in touch with them as well.

Nerves do have a role in controlling acid production in stomach and when control balance is lost , ulcers are produced and are hard to heal.

TENS unit:

It does work for a few people butit is nerve stimulation process and if your nerves don't respond to stimulation , it is good for nothing. I would suggest you to discuss this with your doctor too as an option.

Yes surgical options are always there like Nissen's fundoplication, it usually work for people, but it indeed is weighed by a surgeon keeping in mind benefits and risks,

you can consult a general surgeon about this issue, he might want to run some tests and then he will decide if benefits are more for you.

You can use lowest possible dose of tylenol but dont use cyclobenzaprine if worsens dumping synderome and GERD as it blocks nerves further.

I would alos suggest you to avoid coffee I know you need caffeine but you have to get rid of this habit will take a couple of weeks then your body won't need it much.

Here i want to suggest that Dumping synderome even if doesn't get completely cured, is quite controllable, you just have to understand how it happens, read about it and see what mistakes are you making in your diet andlife habits.

Avoid eating sugar and other sweets such as:

,,Candy

,,Sweet drinks

,,Cakes

,,Cookies

,,Pastries

,,Sweetened breads


Also avoid dairy products and alcohol. And avoid eating solids and drinking liquids during the same meal. In fact, don't drink 30 minutes before and 30 minutes after meals.

Foods to eat. To help with symptoms, also try these tips:

1)Use fiber supplements, such as psyllium (Metamucil or Konsyl), methylcellulose (Citrucel), or guar gum (Benefiber).

2)Use sugar replacements, such as Splenda, Equal, or Sweet'N Low, instead of sugar.

3)Go for complex carbohydrates, such as vegetables and whole-wheat bread, instead of simple carbohydrates, such as sweet rolls and ice cream.

4)To prevent dehydration, drink more than 4 cups of water or other sugar-free, decaffeinated, noncarbonated beverages throughout the day.

5)How to eat. Here are some other ways to lessen symptoms of dumping syndrome:

Eat five or six small meals or snacks a day.



1)Keep portions small, such as 1 ounce of meat or 1/4 cup of vegetables.


2)Cut food into very small pieces. Chew well before swallowing.


3)Combine proteins or fats along with fruits or starches. (For example, combine fruit with cottage cheese.)


4)Stop eating when you first begin to feel full.


5)Drink liquids 30 to 45 minutes after meals.


6)Reclining after eating may help prevent light-headedness.
