Hi.
Thanks for your query, albeit very short and specific.
To recapitulate: Male/53 - medication tried Ibuprofen - no beer, eating less, lots of water, less food - not late at night.

Apparently this looks to be an acidity problem but since your query is so short I would like to know a few more details:
- Since when have you got this problem?
- Is the pain like burning, twisting, piercing or so?
- Any factors that increase the pain or reduce the pain.
- Any bloating, nausea, vomiting, loose stools or constipation?

I would advise you the following:
- First of all, if the pain is acute go to ER and get an EKG s this can be an odd presentation of heart problem and should be ruled out as the first thing.
- If like acidity take PPI like Omeprazole, antacid gel, soft bland diet, get an appointment for Upper GI Endoscopy and further management and investigations of stool, blood, urine, ultrasonography as may be advised on clinical evaluation by your Doctor or Gastroenterologist.
- Getting a proper diagnosis should be the priority as this gives the proper guideline for a proper treatment, its duration and other precautions to be taken.

Pain is only at night. Ibuprofen. Helps pain. Feels like a bed pain that goes away. With a little constipation

Eating at night increases the pain. Mot like a twisting pain

Pain is only at night, increases on eating only at night (not in the day), Ibuprofen helps, feels pain that goes away, little constipation....
This is difficult to correlate as this does not fit in any of the usual diseases. Hence I would advise you the following:
- Clinical evaluation and actual examination of the abdomen and back for spine.
- Mild laxative to soften the stool.
- Consult ER when you are in actual pain so that the actual evaluation is more fruitful and there are chances that the diagnosis can be made.
- Additional investigations may be necessary as per the clinical findings.

If all the investigations are normal, a possibility of IBS can be considered.
