Question: Hi Guys,
This morning I had Open surgery on my left side inguinal hernia. I am now experiencing extreme pain in my left testicle and my spermatic cord seems swollen. Is this normal?

Hi XXXX, I have gone through your question and understand your concerns. Extreme pain in the testicles and swelling of the spermatic cord post inguinal hernia surgery is not normal. It can be due to injury to the testicle or its blood supply during the surgery, or can be testicular torsion, which is an emergency condition.
Sometimes, however, postoperative testicular swelling and pain can be due to the manipulation of the veins near the testes, but such swelling appears 2-5 days after the surgery and can last for for 6-12 weeks.
I would suggest you to consult your surgeon urgently so that you can be properly examined and immediate management can be done, before complications occur.

Maybe its not extreme pain, but it does hurt pretty bad. I don't see any visual changes down there though. The swelling wouldn't appear right away?

Hi XXXX, thanks for responding. As I have mentioned, the swelling of the testicles as a result of manipulation of the nearby veins usually appear 2-5 days after the surgery, and not immediately. Swelling immediately after few hours of surgery can be due to injury or torsion of testes, and should be urgently evaluated and treated.
I would suggest you to consult your operating surgeon as soon as possible.
