Suggest Treatment For Pain In The Scrotum And Painful Urination

Question: Lesions on penile shaft and around anus about 3 years ago. Severe pain on urinating and passing stool. The pain decreased but continue to have pelvic pain and it feels like a heavy load under the scrotum and pain on ejaculation. No pre ejaculate anymore. Ejaculate is thick and chunky unlike what it has been all my life. The severe pain has recurred 3 times but in between there is enough pain that I can't function.

You had not mentioned anything about your sexual activities or sexual preference.
You had mentioned your prostate exam was normal. Whether is per anal examination or ultra sound examination or on investigation on prostatic massage fluid? What about your urine examination? Do you have any frequency or hesitancy during urination. Whether your scrotum examined for the presence of Epididymitis? Kindly send the investigation reports already you had.
Possibility of Prostatitis or epididymitis have to be ruled out.

Heterosexual but can't have sex due to the pain it causes.
Scrotum is not checked.
urine checked many times.
No prostate massage done.

I would like to see the results of urine examination and USG abdomen and pelvis results. You must see a very good urologist or a Venereologist (Genitourinary physician) and get examined and rule out the possibilities of Epididymitis nad prsotatitis. You will be alright soon.

The urine exam was absolutely clear and so was the ultrasound. The question which I asked is still not answered. How to get rid of the acute pain and the chronic pain.

You mean urine deposit has absolutely no pus cells?

Regarding the relief of pain you have to take pain killers, anti-inflammatory drugs and if it is colic type of pain, also antispasmodic drugs, till the diagnosis is confirmed. Only after the confirmation of the diagnosis, a permanent solution can be arrived.
It has to be decided by Your treating physician or surgeon only after careful examination and investigations.
