Suggest Treatment For Misalignment Of Distal And Proximal Fragments In Both Legs

default
Posted on Tue, 25 Nov 2014
Question: My questions is about bone remodelling in an adult of 27 years old. I have had a serious fracture of my tibias on both legs abroad. My legs now are in a good mechanical axis from the AP view, though the distal amd proximal fragments on both legs to not correctly line up. They both misalign by approximately 8mm. My surgeon here in serbia says that everything will correct as strong as before even though there will be an S like shape anatomicaly through the tibias. He also says bone remodelling will eventually perfect this over time. I asked another surgeon and he says this is only true in children. Please give me your thoughts,

Detailed Answer:
Hello,
Thanks for posting your query.
If the mechanical axis is correct then the mal alignment will not have any effect on your posture or daily routine. 100% remodeling is not seen at the age of 27. It is seen only in case of children.
If there is no angulation between the fragments on AP or lateral view then there is no need to worry. This mild mal alignment will provide full body support and strength to the leg.
Gradually this will become less prominent and can be detected only on an X-ray. It will not affect any of your functions.
I hope this answers your query.

Ok thanks
Here are the x rays i have available. The left leg has been mechanical adjusted recently, though you can still see the offseting of the bones. I actualy had tibia varus correction surgery where the original overcoreected the left leg ,combined with a leg lengthening procedure of 5cm in length, The bones do misalign on an angle anatomically though the mechanical axis is ok to within 1-2 degrees. What do you think based in the x rays amd do you think remodelling will help yo improve the S like bending of the bone once healed. I have also included the poat op x rays lateral view for comparison

Brief Answer:
Yes remodeling will help in improvement.

Detailed Answer:
Hello.
Thanks for writing again.
I have seen the X-rays. Callus formation is good. The mild deviation is not worrisome. This will become alright with time. Since the mechanical axis are alright there is no need to worry.
Remodeling will improve this slight bend and you will be able to lead a normal life without any disability.
Hope my answer is helpful.

I appreciate your help. Lastly i am happy that i will have no disabilty. Though please be honest with the following. Do you think i can recover 100% in relation to hard sports etc. or if not will it be close. Also will the misalignments take longer to heal to full bone strength than if i was to get then perfectly aligned. Its just the doctor here says he does not need to do any more, whereas a doctor from the US says it needs fixing but is very expensive. I just dont know who to trust. Please gice your hobest opinion. This is my last question

Brief Answer:
No need of further surgery at this stage.

Detailed Answer:
Hello.
The callus is good but the complete healing will take time of around 4-6 months. At this stage, no further surgical correction is needed. You will recover 100% in terms of hard sports also in case you go for proper and regular physiotherapy to regain the proper muscle strength and full range of movement at the joints.
You are getting the right treatment. Please do not worry.
