Suggest Treatment For Low Testosterone Levels

Question: Hi,

I am 35 years old and I used to have a good sex drive but one fine day when I was under a lot of stress, I lost my libido all of a sudden and since last 3 months I am not able to get my sex drive back. There was some pain in my testes after I lost my lbido which lasted for 2 weeks but went away automatically for which I got the doppler test also done and doppler test on testes was normal. I got my blood work done for FSH, LH, Thyroid etc. and my Testosterone level came very low around 280. Now I am not sure why my body stopped producing testosterone all of a sudden, but I have lost my sex drive all of a sudden. I tried masturburating and am able to do so and semen does come out but there is no feeling of pleasure or sex drive. I am not sure if this is due to low testosterone. I would like to go for testosterone test once more. However lets say low testosterone is cause for this issue, then I have 3 questions.

Q1.) I have heard that Testosterone therapy can cause Prostate cancer. Since I am young and unmarried, is there an alternate way to testosterone therapy that can bring my sex drive back.

Q2. ) Just to see if testosterone is the root cause of the issue, lets say I want to try Androgel 1.62 %, and apply 40 mg of it, my question is that this 40 mg is enough for one day or it will last for one month because I read somewhere 40 mg will give testosterone for one month. May be I misinterprested the text and probably it meant 40 mg everyday for one month but if 40 mg applied once will last for one month then I am worried, since I just want to try it for one day to see if this is the root cause of the issue.
Q3.) Is there a way to bring my body back to natural to produce hormones the way it was producing before. What could have caused the issue.

I think the primary cause for your problem is stress, not low testosterone. you have tested for total testosterone level which can be misleading. you should get a free testosterone level at 8.30 AM from a tripooled sample.
now your questions , generally testes never stop producing it all of a sudden . there has to be a reason for this and severe stress can be one of them
It is controversial but the consensus is that testosterone can worsen pre existing prostate cancer and do not initiate it so before starting it, we get a free PSA level for every patient
The dose is 40-160 mg a day in oral form . The idea is to keep the serum level above the lower normal range. There are long acting forms as depot preparations effective for 3 months if taken by intra muscular route.

Hi, My question is that now I have come out of the stress and it has been more then 2 months but I am not able to get my sex drive back. How will I get married. So whatever damage happened to my sex drive due to stress , how can it be reversed.

Also I am not sure what do you mean by a tripooled sample. Can you please explain.

Also if I get your answer right, 40 mg of testosterone is daily dose and one dose is NOT going to last for complete month. SO I can just test 40 mg for one day on myself to see if that brings my sex drive back. This should not cause any serious harm on me.

Also you did NOT answer one of my question which was since I am young and testosterone may cause prostate cancer although you mentioned it is controversial but still I would like to know is there an alternate for testosterone since I have read somewhere that testosterone replacement therapy can cause prostate problems, I would not like to take risks. Assuming that there is some risk of prostate issues with testosterone, is there any alternate therapy for unmarried people.

One last question, after applying testosterone in gel form (Oral form is NOT recommended since it causes LIver problems), how much time it will take to show its effects.

I usually start my patients on fluvoxamine ER 100 mg a day to nullify the residual effects of stress. Your local doctor will know about this drug.

I have clearly explained that it does not initiate cancer but can worsen it if present. Before starting it if it is low , there is no alternative

Tripooled sample means taking sample three times at 15 minutes gap and mixing the three to get average .
you will get you sex drive back once you follow the steps I mentioned. These are being told after treating around 20000 patients by me. so keep faith , you will be ok

Thanks a lot doctor. The thing is that if this was due to stress then I should have got erection and libido at least in my sleep. However even nocturnal libido is missing.

What I get is, So even if I am completely out of stress and enjoying my life , I should still take fluvoxamine 100g daily. For how many days and can I take it anytime of the day or in the morning.

Also regarding my Test , my Leutenizing Hormone's value came as 1.77 mIU/mL(Range is 1.50 -- 9.30) and FSH Value in my test result was 3.20 (Range is 1.40 - 18.10. My Thyroid was 0.68. My question about my LSH and FSH is although they are in normal range, do you think they are at the lower end for my age which is 35.


So do you think fluvoxamine will bring back my sex drive. I have been trying to watch sensual scenes and lip lock scenes on YOutube but nothing has helped much till now

Ok, Let us say that I want to try testosterone to see if I get my sex drive back. How much 1% testosterone should I try. I have got 1% testosterone 5 mg pouches. Is one 5 mg 1% testosterone enough to deliver testosterone to my body

IT seems you are under the impression that I got the total testosterone done and NOT free testosterone. I had tested myself for both total and free testosterone. My total 375 and free was 2.98 which is very low for my age.

Do you still want me to go for another round of test

My question is that why my body stopped producing testosterone all of a sudden. One morning I was very heavy on sex drive and couldn't control it and the same evening it was totally gone and never came back which forced me to go for this test which means I am very high on testosterone for few days and all of a sudden, overnight my libido and erection is gone, never to come back and my testosterone level also dropped. As I already mentioned in my original question, this was followed by pain my testicles which lasted for a few days and I went for doppler test of testicles which came back normal and the pain also went away automatically.

Also if I start taking testosterone, do I need to take it my whole life now or after some time body will start producing it normally. Also I am unmarried, will my body still produce sperms to father a child in case I want to ??

One more important thing I would like to mention is that before this happened, just to check my self-control I was trying to control my masturburation just to see my self control and it was my 4th month since my control and my mind was continuously after me to masturbate because when you try to control something it hits you more and all of a sudden this happened. Anyway I can get it back naturally.

Causes for sudden loss of testosterone are castration ( if testis are removed due to some reason) otherwise there is never a sudden loss of testosterone production. You are equating loss of sex desire to loss of testosterone which is not correct. Sudden loss of sex desire is generally due to stress which can have immediate effect as loss of desire and long term effects even after recovery from stress due to production of adrenaline which causes constriction of penile vessels so one may experience sex problems even after stress has eased apparently. Pain in testicles can be due to somatic symptom of stress ( I see many such cases daily) . As expected, Doppler study wouldbe normal because we test this after injecting vasodilator .
Lower level of testosterone means your body has lower percentage of free testosterone and if you takefluvoxamine, walk briskly for 30-40 minutes daily in morning , have good sleep for 7 hours and keep your abdominal girth near 36 inches, your testosterone level can revert back naturally so please come out of the dilemma of starting or not starting testosterone . Please start on medicines I suggested and you should be fine in 2-3 months.
