Question: Hello,
I am suffering a very serious issue from last 6 to8 month.My concern is that i have joined a MBA college and my background was science after the gap of two years and in the class room i am physically present but my mind is not working the way i expect what ever the faculty taught in the class i couldn't able to understand anything .After that whenever i try to study in the my room hostel i study but can't remember anything my mind always get diverted whenever i try to study somethings.
I am very afraid to speak in the class whenever i asked by faculty memeber my heart beat goes up when ever i have a presentation which have to present in front of the class my heart bit started beating my voice get chocked i cant able to frame a sentence correctly i always thing what other people will think about myself having said this my spoken english communication is also very weak i rarely interact within the class due to this and also less preparation and bad communication skill. I wanted to study more and more but i cant able to do this. everyone in my class is better than me i am supposed to be the weakest student of our class also i am a very shy kind of guy.
I am trying to study and i did but cant remember my mind tend to go somewhere else.
I completely loose confidence in myself i am really a bad student and a bad human beings noone like me which is why i dont have many friends and also i usually not speak much withothers beause i am a bad communicators. I have to have so many weakness as compared to my others class mates.
Sometimes i am thinking to commit a suicide as i am a complete failure person
So doctor please please help me out i am requesting you to please help me out if you will give me some suggestion i will remain graet ful to you.So please dont mind for my question.

I have read your query and understood your concerns. I have also gone thorough your previous query.

You have been given lot of insight regarding your psychological issues but I want to make a comment here that most of the patients of social anxiety or any other anxiety disorder develop secondary depression which makes the picture of the illness more complicated.

Your major concern is regarding the lack of concentration, not able to think properly and loosing confidence. When a person is stressed with some issues, his mind is repeatedly thinking about it, so when s/he tries to do some work in which he has to give lots of attention and has to concentrate, but because of the stress and rumination about the social anxiety and feeling of inferiority, mind gets diverted and there is lack of thinking and which ultimately increases the stress. So, it is a vicious cycle in which your anxiety and inferiority complex is causing poor performance in studies and this makes you more depressed and further you can not study and finally lose the confidence. All things add up in the end make a depression more difficult to handle and one starts feeling that there is nothing to be done, nobody can help me and it is better to end life. So, XXXXXXX it is depression and it makes a person to think negative, it makes you feel low but actually there is no problem within you.

You may have problem with english speaking but nobody is perfect. DO not consider yourself inferior. These problems will improve when you take antidepressant drugs for adequate time duration. The drugs usually show effect within a month.

For understanding and speaking english you can go to the library of your university and read english news paper, books, you should keep a dictionary with you. You can try to watch english movies and TV serials with subtitles. Nobody is thinking that you are inferior, you have made this assumption that is faulty.

So, visit a psychiatrist and take drugs ask him to give some advice regarding self help tasks to keep one's confidence high. Read inspirational literature.

If you have suicidal thought then you should take a break and visit your home and undergo proper treatment. Please do not act on the suicidal thoughts. Talk to friends and family, they will support you. Problem is only 10%, 90% is how we take it.

Hi Sir,
what do you mean by that i have gone through your previous query i didn't get u properly will you not mind to elaborate a little more further please?
You are oversimplifying my issue here sir whenever there are something new to learn i am not able to pay attention to that subject in the class room i want to study in my room but i cant study why i really don't know whenever i started to open book my mind tends to get diverted to some where else i am not paying attention besides that i think that i have to have a memory problem and the biggest problem which i think is lack of concentration.
I do agree with you on your social anxiety topic i want to learn many thing as much as i can i am badly falling to do why this is happening to me i really cant figure out yet.
Regarding the speaking matter is concern i am keep trying to improve but unfortunately the result is yet to be seen one thing i would like to add is hate to hear my own voice while i am speaking why i really don't know .I have no more passion left in myself to continue something why thing is happening to me really i don't know please help me out .
How to gain confidence and concentration as well as passion will you mind to suggest me because i do believe that Psychiatrist are expert in mental and they do have all the answer to all mental related issues .

I want to ask few questions answer them:
1. Did you have similar attention problems when you were a child?
2. Did you have any kind of learning difficulties like poor in maths or in physics or any particular subject or in reading or in writing etc.?
3. Were you a difficult child to handle? [like repeated complaints from school that you are very mischievous boy, did not sit at one place, interfering with activities of other student, did not pay attention to teaching, missing from the periods in school etc]
4. Is there any history of psychiatric illness in family? if yes then which kind? I am asking for uncles and cousins also.
5. Did you have any stress during childhood? [like divorce of the parents, separation from parents due to job of father, death of any of the parent, physical or sexual abuse, financial constraints when you were a child etc.]
6. How was your performance in school during primary, secondary and higher secondary level? Are you becoming weak student recently or it was there during earlier studies also? For how many months have you been experiencing these symptoms?
7. Did you have these social anxiety problem during childhood also?

Answer the questions, so that I can have better insight into your illness. Answer in the same order for each question.

Regarding the issues with confidence and concentration:
1. Try to make a schedule of activities on daily basis
2. Try to avoid friends that are not very good in studies
3. Make friendship with those whose behavior is good with you and you have opportunity to learn from them
4. Do not read alone
5. If you do not understand something then ask the faculty to elaborate and give example
6. You can have discussion with your friends regarding studies
7. Request your friends to help in understanding the difficult topics
8. Become social. Most important treatment for social phobia and anxiety is to face it, more you avoid it, stronger it develops.
9. Do exercise like walking or jogging in the morning for half an hour daily. It increases endorphin and other neurotransmitters and lift mood which will help you with your anxiety an well as concentration.
10. Do not always engage yourself in studies, you must have time for recreation like outing with friends, going for movies, gossip with them etc. Listen your favourite music in extra time.

These all tips can help you, but important thing here is that you have to take antidepressant drugs, so that you can have maximum benefit.

I fail to understand here that you have the privilege and saw my conversation and gave the same set of answer which another doctor given there is nothing new on your answer which didn't make me satisfy here i asked the same set of question again because i am curious to know more about my issue but very sadly you also gave the same answer nothing extra these social anxiety and all i am very sorry to say to you that it was a copied type of answer . As per as your questions were concerned following are my answer
1.No i didn't have the same problem when i was a child i was a bright student of my school .
Other questions answer will be no till 5th.
6.My performance in the school was good there after it was doping down.
7.yeah some extent in my school and college days i usually tried to remain alone and avoid to mix with other of my batch mates as much as i can.

IF you have any different answer then kindly give otherwise i don't want to listen those copied type answer
i would like to add few things more that from your question No-4 it is clear that you are trying to relate the things into genetic factor which most doctors usually do if they don't have any answer then they simply say it is supposed to be a genetic factor and nothing can be done like (penis size,skin color,height hair some genetic disease) i m fed up listening such genetic related answers given by those doctors.

I feel sad that you do not have any respect for those who want to help you. I have read your previous query because I want to know what has been advised to you and what is your current status. My answer is completely different from your previous conversation. This answers are not copied otherwise the site regulatory body would not provide that to you. They would have notified for my mistake if I had copied it. So, mind your words before writing anything.

I know that you are messed up with your current problems but it does not mean that you have the right to say anything. You are ill currently and that is psychiatric illness. It will make you think in the way, you would not have wanted at any time in your life.

Regarding your main query: The treatment of depression and anxiety symptoms is drugs and psychotherapy with modification of life style, so when you ask to any psychiatrist regarding the management of your illness, the answer would be similar always. If you have been given the same answer, it means I am also right and you need to do something about what has been suggested to you. Your illness is not improving, so it is not doctor's fault. There are 30% of the psychiatric illness or even the anxiety and depression that are complex and requires long time of treatment before remission. Some psychiatric illnesses are classified as resistant and requires even 3 to 4 drugs at the same time with psychotherapy can take 2 to 3 years before showing improvement. These details will not be told by any other doctor, I am telling you because I want to show you that illnesses differ from person to person.

You have asked regarding the different answer, so I am giving my last answer: So, if you are not taking any drugs then take proper treatment in terms of antidepressants for at least 1 year.

I feel that your thinking has errors based on faulty assumptions regarding how others are thinking about you. You have accepted yourself as inferior to others, which is the most important factor in your symptoms and problems. In my opinion you would benefit from cognitive behavior therapy.

I was asking regarding ADHD in childhood because you have major complain of attention deficit. Most of the patients of childhood ADHD have the attention deficits during adult life.

I have asked regarding family history of psychiatric illness because Depression runs in family and same is true for anxiety also. If you have the family history of schizophrenia then I would have questioned your primary diagnosis because you might have some complicated psychiatric illness which would require thorough evaluation and proper management with in person interview with psychiatrist.

According to the answers you have given to my questions, I would say that you do not have adult attention deficit problems. So, the symptoms you are currently facing are because of your anxiety and depression secondary to anxiety. Treatment and other things you know very well because you have been advised so many things, so I don't want to repeat that.

I feel that I can not help you more than this but if you really want to fight this problems then stop writing through internet and go visit a psychiatrist and psychotherapist in person. If you do not have psychotherapist in your city then nobody can do it online. You can read regarding cognitive behavior therapy online and try to modify your thoughts.

Hi Sir,
I am extremely sorry for what i wrote and it did hurt you so i am seeking apology for that my intention was not to hurt you it was not deliberate.
As you know the problem which i am facing is ruining my life i am not able to overcome it .?
Like i said before i am facing serious problem in terms of memory, loss of concentration, not able to focus on study but i m desperate to do well in study but i cant do this which gives me a lot of frustration my life is becoming hell day by day.
I have visited a psychiatrist nearby she gave some medicine like inderal 40,feliz-5 but unfortunately no result has been seen yet so can you suggest me a good psychiatrist in XXXXXXX Ncr region .
Sir i have no patience to wait 2 to 3 year time which you mentioned in your reply.
sir You correctly point out may be this could be reason i always think what others think about myself when i am not able to speak English or answering question even if i know some question answer but i am afraid to speak due to fear.

You drugs are in low dose. If you were my patient then I would have given you Inderal TR 40 mg in the morning with Feliz Plus 10 mg at night. Since you major concern is regarding attention and concentration, I would have given you Tab. Inspiral [methylphenidate: which is a crystal meth kind of drug that is used for ADHD in children. It increases attention and concentration. It is not available easily and you need to have a prescription for it from a psychiatrist]

We have good psychiatrist in XXXXXXX Dr Sheikhoo Bisnoi [who has answered your previous query] lives in XXXXXXX and has a clinic also. Read his profile and find the address. You can meet him at his clinic during the visiting hours. He will manage you properly. I feel that this would help. CBT will be the most effective for you. Inspiral can show significant benefit to you but it is not available easily.
