Suggest Treatment For Itchy Skin Lesions

Question: Thanks for reaching out to me doctor for the update. As you know I have been taking the medicine as advised by my doctor and I am due to see her again in 2 weeks. The new growth of affected areas on skin has stopped, but the ones which were affected have not improved. Just the itchiness has gone but the dark spots remain. I am hoping that the dark spots will go away soon.

Brief Answer: Reduction of itching is good sign Detailed Answer: Hello XXXXXXX Welcome again to the forum. I am happy to know that your new growth of affected areas on skin has stopped. This shows a good prognostic sign in view of halting the activity of your skin problem. Reduction of itching is one of the most important points in reducing the problem in Lichen planus. It is the itching which is most troublesome and is the main cause of formation of new lesions. Once you scratch the area there is formation of new skin lesions in lichen planus. Therefore you have to take care of the itching at your best. I guided you in my previous XXXXXXX that scratching induces the formation of new lesions. So reduction of itching should be considered an important point in your skin condition. Do continue oral antihistamine therapy which provides the best relief without any side effects. For the areas where you see less improvement- you can apply the cream for two times and then give a gentle massage (exactly over the affected area). This will improve the penetration of the cream into the affected area of skin. Once you see that there is reduction in activity then you can apply singly or on alternate day basis. For the part of dark spots- It takes bit longer of time to get reduction in pigmentation. In my patients once the activity is halted I generally start with certain skin lightening agents such as Kojic acid or azelaic acid or hydroquinone creams. But these are started only if the activity of formation of new growth is halted. You can ask your doctor if she can start with one or the other of good lightening agents as per her clinical judgment.
