Suggest Treatment For Irritation And Bowel Leakage

Question: I think I may have a hemorrhoid - I feel something is "down there" and have been having an irritable pain and some (very) minor stool leakage, especially when I am running/jogging. About 3 weeks ago I had a course of antibiotics for 5 days after a cosmetic procedure. Preparation H has seemed to help but I am concerned about the leakage. What are your thoughts? THanks so much...

I have gone through your medical history carefully and understood your concerns.

1. A photo of the thing you feel down there is required for further evaluation.

2. Antibiotics use is associated with altered intestinal flora and changes in bowel movements.

This is what happened to you.

Probiotics can help restore the normal flora and improve the symptoms.

Prep H is encouraged to apply locally as it will improve local irritation.

3. With regards to leakage which is more notable during physical activity like joking; it is related to difficulties you have gone through when passing the stool (irritation of anal mucosa).

To make sure that the anal sphincter is ok ( as this is a new problem to you), I advise to get further examined by proctologist.

Kegels exercise will help restore the normal activity of the muscles in pelvic floor.

A stool culture is recommended to rule out Candida overgrowth and get it correctly treated.

Thank you for your answer. Can stool leakage be related to the hemorrhoid?
And, I have had a stool test about a year ago which diagnosed Candida..
Would that cause the leakage? How does "irritation" of the anal mucosa occur?

Hi back,

Thank you for providing additional information.

1. You are reporting a previous history of candida infection. When using antibiotics, it has also associated with candida overgrowth. Antifungal treatment is advised (e.g. nystatin )

2. If you find it difficult to pass the stool, the harden feces would lacerate the anal/rectal mucosa. This laceration will end up in inflammation and fecal leakage.

Furthermore, you might be straining alot in order to pass the stool. This would have altered the normal function of muscles in the pelvic floor leading to leakage.

That's why should run rectal examination to evaluate the function of anal sphincter. Kegels exercise can also help.

I am now having the bowel leakage again. This time related to loose stools and when I am running/jogging. I am concerned that this a implies a serious problem. My question is would loose stools or a probiotic be causing this?

Sorry for the late reply due to my XXXXXXX clinical overload.

1. You are reporting to have leakage again after passing loose stool. Leakage is present even in cases where your bowel is not emptied totally.

2. The very first thing to do is to see if anal sphincter is ok. That's why I highly recommend to consult a proctologist and run above mentioned tests.

3. I do not think that there is a serious problem although I still think you need further evaluation.
