Suggest Treatment For Insomnia

Question: I rarely get sleep at night normally I fall asleep around 5 am and wake up around 10 am since last 10 years.I can get sleep anytime during the day, but feel difficulty in night, even if i have passed 24 hours without sleep, but do not fall asleep before 5 am. As a result, i often could not get sleep for a single minute at night before the exam as the exam starts early in the morning. It impacts my performance in examinations, please suggest a remedy.

Thanks for writing to us. I can understand your concern.

Your body time cycle (circadian rhythm) is different from others and that is why you are facing the problem. You should try to change it gradually for overall benefit in life. You should try to sleep at least before 2 am. You can do it same by gradually awakening early for half an hour and next day you may be able to sleep half an hour early. Or you can pass 2 days without sleep and third day you will be able to get sleep early. With practice you can change your sleep cycle.

Many students do not get sleep during exam days and many times they are prescribed medicine with successful results. For a while if exam is near you can take help of medicine. Zolpidem is the medicine which helps in sleep induction and makes the person sleep fast. You can take the same between 10 pm to 1 am according to need. It will help you in sleeping. You will not have sedation after waking up and you can awake easily and perform well in sleep. You may have headache as a side effect and if it is there you can take paracetamol for that.
Zolpidem is a prescription medicine and you need to take the prescription form your family doctor.

I have tried to change the body time cycle by avoiding sleep for 38 hours and sleep by 12:00 third night and woke up around 5 am but I feel sleepy full day and yet follow routine for more than two months with difficulty. In the last I had to give up and my cycle return to old pattern in one day.Other than odd timings of sleep ,I face no other problem.

You have circadian rhythm disorder. This can be treated by medicines and psychotherapy.

In medicines melatonin, zolpidem or benzodiazepine group drugs (clonazepam or lorazepam) can be used with alterations in sleep timing and daily activities. In psychotherapy you can be helped how to change your schedule without stress. For this detail history and few sessions are required.

Consulting a Psychiatrist will solve your problem. You can do this after your exam is over.
