Brief Answer:
poorly controlled diabetes

Detailed Answer:
Hi,
Welcome to HCM.
You have very poorly controlled diabetes. With the present medication glynase , it is difficult to get good control.
First, you need to keep strict restrictions on diet. Regularly exercise for at least one hour a day. Avoid sweets or sweeteners. If you are stressed either by occupation or other, try to keep away from stress by meditation.
If all these measures fail to correct your sugar levels, you need change of drugs and may be insulin also which will be decided by your doctor.
I would also recommend to get kidney function tests on regular basis.
All the best.
