Question: I have a friend who is a Vietnam war veteran; male age 55 I think. Hands tremble, some memory loss, some balance problems (also has a bad knee) suffered several concussions playing football and some schrapnel
in arms and legs.

I have gone through your question and understand your concerns.

These symptoms are not specific to a medical condition. However it suggest that these could be related to the brain and require extensive examination and investigation to reach to a diagnosis.

Since the symptoms are not pointing towards a diagnostic condition, I would have to rely on clinical examination and investigation. Some investigations which may be done are brain Mri, blood sugar, Thyroid profile, vitamin B 12, renal function test and liver function test.
Nerve conduction study and imaging of spine may be required if clinical symptoms and sign suggest so.
Vietnam War is linked with agent orange related diseases like parkinsons, neuropathy and cancer. I would suggest that he consults a neurologist to evaluate on these lines.
