Question: i have already consulted with you before.i am taking now monit sr 30, prolomet xl 50,deplatt a 75,lipicard 160, i uploaded my angio report which was done in 2007.please clarify my CAD situation and its hazards.till 2007 my lipid profile is in limit range and creatinine,blood sugar , are not crossed the limit range.some times mild chest pain occurs.i gave up smoke some days before because when i smoked chest pain occured.now after giving up smoke it is not occuring like before.please sir suggest me if angiography is needed now ?
and suggest me how should i change my lifestyle in which the blockage does not increase.
my sgpt,sgot was high so i did not take lipicard 160 for 3 months. now i am taking this 7 days is going on.now sgpt is 60,and sgot is 58. can you suggest me any better medicine instead of lipicard 160.
i have grade ii fatty liver and non-alchohilic.
also i was taking amixhide-h during 8 months for headache,anxiety, is it needed now?

You need to recheck

Considering you had 50% block in 2007 you need to recheck but before going for angiogram if you are able to walk on treadmill better to have treadmill done. If it shows signs of ischemia you need to go for angiogram. Otherwise you can continue to be on medicines and recheck regularly about once in month.
With regular medicines and lifestyle there is only 30-50% remains unchanged in remaining it progresses albait at slower pace

The cholesterol medicine is chosen based on lipid profile. so you need to continue it till next lipid profile about 3-6 month. in case you are having side effects than we may change. Please let me know what's your expectation when you say better medicine. Also if you want to change do a fresh lipid profile and send across

What are the values?

Among the medicines that are given for the cholesterol Finofibrates have least toxicity. If your enzymes are raising with finofibrate than we may not be able to use anything else also.

If they are raised less than 5 times from baseline and absolute value is less than 200 than we can continue to use, while monitoring the liver enzymes closely say once in month or you develop symptoms like anorexia. if the values are higher than better to stop medicines ..
