Brief Answer:
It is a complicated urinary infection

Detailed Answer:
Hello. Thank you for posting your question on Health Care Magic
First and foremost please get your urine analysed for infection if found positive send it for culture.
After certain age body doesn't respond with fever but the infection ascends up without being symptomatic
Iam suspecting that you could be having pyogenic spondylitis or a renal stone Please get an Ultrasound of Kidney and MRI of your back
Mean while for the severe pain you can start with Diclofenac sodium or ibuprofen
If urine analysis suggests infection start off on antibiotics, but before starting give the urine for culture
Best wishes and speedy recovery
