Suggest Treatment For Frequent Acne

Question: Hello sir/Ma'am,

I have been suffering acne from when i was on 16 years now i am 26 years still the acne occurring frequently .I have visited numerous Dermatologist but unfortunately that was proved to be ineffective. I have tried azithromcyin ,clindamycin, benzoyl peroxide and salicyclic acid based face wash and i had also taken 100 mg of isotretinion tablets for 3 months as prescribed by the Dermatologist but those were also in effective when i was taking treatment the acne tends to get disappeared but when the course is get completed within few weeks the acne came out.Here i failed to understand those Dermatologist knowledge to treat acne. I am also doing frequent masturbation which is why the acne are coming because the hormonal fluctuation which i think.how it can be possible to have acne at 26 years of age?

I have a very few questions which are as follows:

A-Why acne occurs?

B-Is there any relation to hormonal level ?If yes then at the age of 26 what do you think that hormone get fluctuated as far my knowledge is concern it is get fluctuated on puberty stage the fluctuation of testosterone hormone.at 26 years of age is tends to negligible?

C-In my family why i only suffering from acne besides me no one ever get pimple?

D-What are the permanent treatment of acne beside which i have mentioned?

E-What are the checkup which has to be followed to detect why the acne are occurring frequently?

F-My skin is oily which cause acne is this true i don't thin so oily skin doesn't cause acne am i right?

If you have the answer then please give in a details manner don't try to give the typical Dermatologist way i am expecting something more because i need to find out why the acne are occurring frequently on me is the any problem inside my body?

Thanks for the query.
Acne happens due to blockade of sebaceous glands.
It happens in following steps.
1. Sebaceous glands produce lot of secretions in acne prone people.
2. This sebum gets mixed with dust and causes blockade of sebaceous glands. This stage is called as comedones.
3. These comedones get infected by bacteria and lead to pus filled acne lesions.
Acne formation is not related to testosterone or masturbation.
It has relation with the following.
1. Oily food which adds in to sebum production.
2. Fast food like pizza, rich in carbohydrates which leads to increased level of insulin in body and induce gland blockage.
Here I am describing the entire procedure to be followed by you.
1. In evening take steam and wash your face with tea tree oil containing face wash and apply a combination of adapalene and benzoyl peroxide gel.
2. Early morning apply clindamycin gel.
3. After bath apply a water based sunscreen.
4. At around 12 pm again wash your face with tea tree oil face wash.
Do this for a month and see response.
Again is not hereditary so your family members need not be affected.

Hello,
I did a lot of secondary research on acne.What kind of response you are talking about.I know those doctors very well they kept on busy treating with disease by giving the medicine which they but they never ever try to find the root cause of the disease that why these things happening on a constant basis.
I am not seeking the treatment here i am trying to find out the root cause.
You gave the google type answer about what we call acne.I think you didn't read my query properly if u did then i have already mentioned benzoyl peroxide along with that Retinoid gel besides that i have already taken Retionid like isotretinoin.
As far as your oily food is concern i usually tends to avoid such food i am very much diet conscious and i also wash my face regularly three times a day with Aloe vera, Glycolic acid, Vitamin E based face wash called AHAGLOW,along with that i used salicyclic acid based face wash,Glogeous face wash,cetaphil face wash too none of those proved to be effective.

This very sad that you haven't answered all my question i do think you didn't read my query properly.
Do you give the confirmation that if i will follow your procedure then i will never have acne again as said in my previous query that i have visited numerous dermatologist and tried everything but result is yet to be seen. Before answering the query first read it thoroughly.

As you mentioned about sunscreen i have already used Ansolar,Photoban 30,Suncross aqua gel and kept on usinga ansolar also used adapalen 1% combination gel, along with that i have used triple combination cream for pig mention(hydroquinone),kojivit cream,moisturizer like cetphil,Temptin, along with that vitamin c for oral suspension with vitamin c serum.
This Acne render my life to crippled hence please do clear my doubts if u know more.

I have read your question very well and answered.
I am sorry that you were not happy with my response.
I would try to explain it to you.
There are certain drugs which can be used in a particular disease, same goes for acne.
This is the reason why dermatologist have been prescribing you adapalene, benzoyl peroxide, clindamycin etc.
You asked about the root cause.
I explained you step wise how acne is formed what else information you need.
There is no single root cause which cause acne.
Acne is not some external infection which can be totally eliminated.
It is formed from within the body and hence keeps on appearing.
Do you want me to explain till molecular level I can do that if you can understand.
There is no impurity or any such thing in blood leading to acne.
Regarding treatment advised by me, I have given you a complete chart how and when you have to use which medicine.
I know you have used all the medicines and these are the medicines available for acne in entire world.
There is a prescribed way in which the medicines are to be used.
I háve tried to explain you the same.
I request you to follow these steps and see if it works for you.
Tea tree oil is a good anti acne agent and steam opens the clogged pores and reduces the chances of comedones turning into acne.
There are steroid based creams which totally suppress acne and make face clean but slowly damage the skin.
I hope you don't want to use them.

Hello,
Before proceeding further what do you mean by that " Do you want me to explain till molecular level I can do that if you can understand?
Let me tell you I have studied science with biology so i can understand.
What do you mean by that there is a prescribed way to use drugs are you trying to say that the others Dermatologist who had prescribed me with those drugs was not the prescribed way?
Do you want me to believe that if i will follow your one month treatment then acne will be minimized as i had been using your prescribed drugs for 2 to 3 years besides your steam and tea tree oil face wash?
Now coming to the cause which you explained was not complete have you explained about hormonal factors, have you explained about stress, have you explained about sleeping pattern? Acne is a multi factorial as you quoted was right.
You quoted acne formation is not related to testosterone so now coming to over production of sebum by sebaceous gland and the vital simulator of sebaceous which produce more sebum in male are androgens including testosterone may be i am wrong before i will proceed further about another aspects of acne i want to see how you explain acne till molecular level let me see first so that i can learn new things from you.
As i said before i did secondary research on acne which is why i have many documents about acne.So please explain from molecular level let me see how you will explain after you explanation i will proceed .Please don't talk about treatment i will not suppose to follow your advise or anythings as i know these things are not effective in my case as you didn't advised me somethings new.

It's great to know that you have done complete research.
First of all testosterone is not the cause for acne else every male in this world would have been suffering from acne.
You can get your testosterone levels checked even that will be normal.
Few people have increased responsiveness to testosterone and that's what lead to acne.
There are receptors on sebaceous gland which convert testosterone to its active form.
Excessive responsiveness of these receptors is the cause for acne and not the testosterone itself.
Stress leads to increase in cortisol levels which in turn increase levels of igf ( insulin like growth factors) which enhance the epithelial proliferation of sebaceous gland duct leading to blockade.
Any thing which causes increase in igf will cause acne.
As I mentioned earlier initially comedones are formed which get infected by p acnes leading to pustule formation.
Excess carbohydrates cause increase in igf so I asked you to decrease them.
Tea tree oil has good antibacterial effect on p acne.
Adapalene and other retinoids modify the epithelial proliferation of duct by acting through TGF beta and EPF and hence maintain the duct in open state while benzoyl peroxide is an effective antibacterial agent.

Hello,
I would not like to go with much more details on acne cause as of now as i am not a doctor and i would not like to disrespect you or contradict you which i can.
Suggest some expensive and effective drugs or gel or cream not the composition which i know very well.
I have lot of acne scars on my face suggest some treatment about that as i know 80 % acne scar can be minimized by various kinds of laser procedure i am not going with details about that as i am not the right person to do it is the Dermatologist who can decide.
Suggest some expensive and effective treatment of acne post hyper pigmentation sun XXXXXXX on face how to improve the skin texture quickly as my skin texture doesn't match with my body color.
For your better assessment i have attached my face photo graph may have a look and suggest the best you can.
I am expecting something more kindly try to help with same the best you can not that typical way i want the expensive as well as effective treatment.

I can understand your concern.
I know acne causes lot of disturbance due to aesthetic concern.
The photograph shows that you have few deep scars in cheek region and pigmentation on forehead region.
You can go for dermaroller for cheek and glycolic acid or mandelic acid peeling for forehead region.
There are no fresh acne lesions at present so dermaroller can be done now.
I cannot suggest you any creams as all the anti acne creams contain adapalene, benzoyl peroxide or clindamycin.
Its better for you to go with chemical peels and dermaroller.
Chemical peels help in removing pigmentation as well as comedones.
LASERS can cause post inflammatory hyperpigmentation hence better to be avoided.
