Question: I am a 34 yr old mother of 4. I have wonderful husband with a good job And we have a great life. But I have been becoming more and more forgetful. My husband never asks me to do anything for him anymore because I always forget. Then I began getting very emotional, crying everyday. So I saw a doctor, started taking citalopram, and seeing a psychologist. I felt a bit of improvement, but in the last month I have been getting panic attacks. It's always when there's a lot going on and I feel out of control. I have been having a hard time controlling the tears and what comes out of my mouth. I get angry and overwhelmed very easily. When I get overwhelmed I don't feel like I can get anything accomplished around the house- it's always a mess. Then I get more overwhelmed. I have to get myself out of this rut! Help!

Mostly Bipolar disorder with ultra mood swings

I can understand your problem.
You have given a very nice and complete history. It is easy to diagnose for me your illness.

Actually you are having bipolar disorder (BPD).
You are emotionally labile and suffer ultra mood swings.
Your illness is curable.

Your forgetfulness is pseudo dementia that means due to depression. So after taking antidepressant problem of forgetfulness will go away.
Your doctor has given medicines to overcome depression.
But you also need a mood stabilizer drug like divalproex sodium or lithium carbonate.

It is good that you are doing counseling sessions too side by side. it will help you to manage your anger.

In my opinion treatment should be

1. Tab divalproex sodium delayed release.
2. Continue escitalopram but dose should be 10 mg.
3. Tab methylcobalamine 1500 mcg
4. Tab olanzapine
5. Tab clonazepam

Review after 14 days.

You will be fine in 2 weeks after taking these drugs. So do not worry.
Your treatment will be continued for 2-3 months with some counseling sessions.

As these drugs are available on prescription only, so visit your doctor again to prescribe these drugs.

Wow I was not expecting that. I don't feel like I have many if the symptoms of mania, like nonstop talking or big bursts of energy, or feelings of euphoria. I'm definitely an emotional eater, I like to shop but I don't make way over the top purchases or anything. I do feel like I am incredibly indecisive. I guess I just feel like I have times when I feel "normal" and have the energy and drive to accomplish what I want to, but that usually is only 1 day every couple weeks, and then im back to feeling sad overwhelmed etc.

that's why I used the term ultra bipolar. mood swings for very short period.
BPD doesn't mean full mania phase. some times depression with normal or hupomanic phase.
do not worry visit your doctor. take these medicines in smallest amount and go for counselling sessions you will be fine in 2 weeks.
do not forget to take methylcobalamine vitamin.
