Suggest Treatment For Fibromyalgia With A History Of Lyme Disease

Question: Hello Dr. Rynne
I have Fibromyalgia with a history of Lyme Disease...... I take Ultram Flexerill and Lyrica ..... I take 150mg of Lyrica twice daily ......... Im needing to take it more often now ...... Without it Im in a lot of pain ....... I told the doc who is working with my Neurologist ...... His reply is that Im on a high dose of Lyrica and he wants me to see a Rheumatologist.......... Do you know if in the state of NJ I can have my Lyrica refilled five days early ? I will discuss this with doctor the next time I see him but as for right now Im out of my Lyrica and in pain ......... I got it filled last 25 days ago..............

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that lyrica contains pregabalin and that will not be available as on the counter drug , its a prescription drug only .Yes thats a high dose too .

My suggestion would be start you on amitryptylline rather than lyrica as it has got a better action in pain modulation and is the one recommended for the same . Till the time you see your doctor you can take ultram twice daily , it should effectively control your pain .

Hello Doctor,

Thank you for your reply ........... Is Amitryptylline an Anti-Depressent ? If so I believe Ive had that medication with intollerable side effects ....... Lyrica has some not so pleasent side effects as well but it is helpful ......... My pain level is very high. I would say on a scale of 1 - 10 pain level is a 7 . For years I have managed my pain with Ultram / Flexeril and especially over the last year Lyrica Along with yoga, stress management, supplements, diet, strength training, music and outdoors walking in the sunshine . Also Gatoraid regularly and salt tablets as needed for Orthostatishypotension ( that came along with the Fibromyalgia )......Having said all of the above ......... I have refills left on my Lyrica Rx ....... What I want to know is if my Pharmacist is allowed to refill my Lyrica today 12/5/2014 if it was last filled on 11/10/2014 in New XXXXXXX ......... My insurance said they will pay I just want to make sure the rules in NJ will allow my refill today ....... I don't use medications stronger than the ones Ive noted .... I feel that Doctors and Pharmacisists and patients are being bullied by new laws that are to reduce Rx drug abuse ...... Drug abusers are not going to want my Lyrica Ultram or Flexerell....... If you don't know the laws in NJ do you know who I could call ? I don't want to call my pharmacy and be treated like a drug abuser ........

Well yes amitryptylline is an antidepressant and if it doesnt suit you there is no point in recommending that .

Regarding lyrica refill if you have your refills left then in my opinion he should allow you that although the drug policy is different in different states but they cant see you as a drug abuser as you have been diagnosed with fibromyalgia .

I think you should call your pharmacist or your doctor as only these two people can exactly tell as it depends on your prescription also what your treating doctor has written in that as the exact dosing requirement differs from patient to patient. And its your right to ask them so go ahead and enquire about it.

Thank you doctor, I will do that I will also follow up with a Rheumatologis ..... I have to admit that Im concerned ...... I have plans for the second half of my life and altough Ive seen Rheumatologists in the past for my Lyme and even Fibromyalgia all agreed to treat with Ultram and Lyrica plus the drug you suggested ..... My concern this time is that my Rheumatologists dx is going to be different ...... Im concerned that it will be Rheumatiod Arthritis ( my grandmother has that ) or Osteo Arthritis or some type of Lupus ( my aunt has lupus ) something else that will prevent me from having an active life ( well as active as my life can get with a history of Lyme and Fibro ) My finger joints look a little unusual slightly twisted . Are there drugs to stop the pain, damage and disfigureation Rheumatiod Arthritis causes? ........

You should see a rheumatologist soon as all the joint related and autoimmune disease have a genetic inclination . And that ways you have a strong family history. Rheumatoid arthritis cant be cured but it can be very well controlled and the deformities can be prevented if the treatment is started within time . Methotrexate is the drug of choice in that case .

You will need to get RF/Anti CCP/ANA tests done for the diagnosis of the same . But you should not delay things as timely intervention may help in preventing progression.
