Brief Answer:
IBS is a diagnosis of exclusion,It can never be initial diagnosis.

Detailed Answer:
Hi
Thanks for asking your query on this forum.
Have gone through your details and i appreciate your concerns.

See from your narration it seems that there is some problem with patency of rectal muscles also termed as fecal incontinence. Fecal incontinence is the inability to control bowel movements, causing stool (feces) to leak unexpectedly from the rectum. I agreee that urge to defecate can be due to irritable bowel syndrome but dirtying pants is suggestive of some problem in muscular strenghth.
In old age Nerve damage or Injury to the nerves that sense stool in the rectum or those that control the anal sphincter can lead to fecal incontinence. Second most common cause is weakening of muscles in rectum and anal canal.

Initially i would recommend you to go for in person digital examination of rectal canal by general surgeon.A finger will be inserted into your rectum to evaluate the strength of your sphincter muscles and to check for any abnormalities in the rectal area.

Magnetic resonance imaging (MRI). MRI can provide clear pictures of the sphincter to determine if the muscles are intact and can also provide images during defecation (defecography).

Irittable bowel syndrome is a diagosis of exclusion when all other possibilities are ruled out patient is labelledas having IBS.Your symptoms like crampig and urge to pass stool can be due to chronic gastroenteritis also.
Get a stool microscopy test done to rule out this possibility
An abdominal ultrasound should be done to rule out any other possible problem.

If every thing turns up to be normal then we should label it as IBS and treat it accordingly. Please discuss this with your gastroenterologist.

As of now start practising kegal exercises and follow the diet as suggested below.

- Eat plentiful "fresh oranges" and "pear "
- do some exercise everyday as per your capacity try to take a 20 minute walk.
- Don't worry too much.
- Take a diet which is very low in Fat.
- Add lot of vegetables and fruits to your diet.
- Fruit juices should be taken.
-Take two tea spoon isphagula husk with 200 to 300 mL water once while going to bed every day.
