Suggest Treatment For Fecal Incontinence

Question: I have fecal incontinence. I have been diagnosed with IBS. Adding fiber gives me diarrhea. Eating fruit does the same. I have improved somewhat by eliminating sugar. Eliminating dairy and wheat does not seem to help. When I get diarrhea I have no control. I am pretty much homebound.

Hi,
Thank you for contacting on HCM. Since dietary changes havent helped you much especially in the face of diarrhea episodes, next step would be the use of antidiarrheals only at the time of diarrhea episodes.

1.Diphenoxylate hydrochloride 2.5 mg with atropine sulfate 0.025 mg

This drug combination consists of 2.5 mg of diphenoxylate, which is a constipating meperidine congener, and 0.025 mg of atropine to discourage abuse.
2. Loperamide

Loperamide, which is available over the counter, acts on intestinal muscles to inhibit peristalsis and to slow intestinal motility.


Alosetron is your last option if your diarrhea doesn't get controlled with anything else. Although its very effective, however its use is discouraged unless having very severe symptoms because of its numerous GI adverse effects

Please know you can not use any of those medications continuously for long term, they are only for temporary relief best used only at the time of diarrhea episodes. In some cases Psychotherapy referral can also help with this condition.

I am on warfarin because of Factor V Liden, hydrochlorothiazide and lisinopril for HBP, levothyroxine, pravastatin,citalopram ropinirole (restless legs) and 65 mg of iron daily. Vit D, a Mega Red with Krill Oil. About 1 year ago I was diagnosed with IgA nephrology. Will of those meds interfere with the anti diarrhea meds?

Hi,
The antidiarrhea medications are not interacting with your other drugs. However I noticed with the use of levothyroxine and warfarin, there is a serious interaction. You need to replace warfarin preferably with rivorxaban which is a little more expensive but would be safer. Warfarins level can increase to levels with levothyroxine to dangerous levels, so a replacement is recommended by international guidelines.

I have already answered your question previously. You may go to your GP and inform him of the serious interaction between warfarin and levothyroxine and ask his opinion. I suggest you may go on rivoraxaban.
