Suggest Treatment For Fainting Spells When On Lisinopril

Question: My mother was in the hospital related to what we thought was a possible TIA that might lead to a stroke since she exhibited all the symptoms. She stopped taking her labatelol since she was having fainting spells and not feeling well. She is now on Lisinopril daily, Metopralol daily, Hydralazine TID and PRN qid. She also takes Crestor. They were having difficulty getting her BP under control and we are still having difficulty now that she is home. The medications will decrease her systolic and dystolic but will elevate at some point and time and when given the PRN Hydralazine, it will decrease her systolic but her dystolic is still in the 80's and 90's. She is not taking any diuretics but we are having to tell her to try and drink more water because she will not drink if not encouraged to do so. We are at our wits end. She is 85 years old. She also takes ASA and MVI.

I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:
1. Diastolic of up to 90 is normal at this age. Systolic pressure of XXXXXXX of 140 is good.
2. Hydration is very important. She will require multiple feeds and also fluids in the form of fruit juice, milk, curds etc. Avoid coffee, sodas and other dehydrating agents..
3. Blood pressure would take little time to stabilize after hospital based injectable therapy. Not to worry, until its less than 140/90.
4. Mild physiotherapy, exercises and protein diet will be helpful in improving immune system. As she is recovery, she will require constant care, otherwise, cognition deterioration would start.

Family support and complete caretaker management is necessary for at least one month. Then she could be able to manage more easily.
