Suggest Treatment For Dry Cough And Fever In Children

Question: FOLLOWUP -
Hello Doctor (below) is the original question

Doctor; it is now 8 days our daughter has had a fever ranging from 100.1 - 102.4
Her cough is a little more productive - but still mostly dry
She is active, Eating, and drinking and in good spirits - no other symptoms

We are concern? Should we be concern?
We have never had a child have a fever for 8 days.

BACKGROUND:
My 10 year daughter has had a dry cough for 4 days
fever ranging from 101 - 102 degress
today is day 5 - now cough is a bit more productive - clears her throat alot
a little fatigue
no other symptoms

2 doctor visits - no action taken by the doctor - says its a cold
at my daughter school - there is a alot of phemonia and flu

my daughter had the flu shot in Oct 2014

I am worried - the fever has lasted for 5 days
yes I am alternating between tylanol and advil

Hi...I went through your daughter's history and the medication history too.

I have a few questions for you -

1. How is her activity in between 2 fever episodes on the same day?
2. How is her appetite?
3. Has a blood culture been performed before starting the antibiotic Amoxycillin?
4. What is your daughter's weight?
5. Apart form fever - does she have any - swelling in the neck/ puffiness of eyebrows or eye lids/ abdominal distension/ altered sensorium / diarrhea/ pain or burning sensation during voiding urine/ unbearable cough/ respiratory distress?
6. Has a chest X-ray been performed as we both had decided last time - if so kindly upload the image?
7. If any blood test reports or urine test reports are available - can you scan and upload them?
8. Does she have any white patches on the throat and tonsillar region?

Suggestions on possibilities -

1. Prolonged viral illness.
2. Enteric fever or Typhoid.
3. Atypical pneumonia getting complicated.
4. Ebstein Barr viral infection.

Kindly get back top me with answers to the above questions and I will be in a better position to guide you.

Cough Productive - She says feels like something wants to come out, but little does

8. Does she have any white patches on the throat and tonsillar region? YES, she has white dots or patches in the back of her throat - throat is pink not red

1. How is her activity in between 2 fever episodes on the same day? Active, happy

2. How is her appetite? Good, not as good as normal, but not bad, I would say 75% of normal

3. Has a blood culture been performed before starting the antibiotic Amoxycillin? No blood test

4. What is your daughter's weight? 61 pounds

5. Apart form fever - does she have any -

swelling in the neck - I felt the back of her neck and under her throat, no swelling

puffiness of eyebrows or eye lids - no puffiness or swelling of the eye lids

abdominal distension - no abs are flat

altered sensorium - her intellectual and cognitive functions are normal, she is performing homework as she is not allowed in school with a fever

diarrhea - No

Pain or burning sensation during voiding urine? No; she went to the bathroom 20 minutes ago and felt no burning

Unbearable cough - unbearable coughing No, constant coughing yes,

respiratory distress? None

6. Has a chest X-ray been performed as we both had decided last time -? No I took her back to her doctor - dr. office visit said her lungs were clear, no presence of phemonia -

7. If any blood test reports or urine test reports are available - NOne



Suggestions on possibilities -

Ebstein Barr viral infection -

Her older sister got very sick before her, fatigue, under her throat and behind her neck were very swollen however we had her tested for Ebstein Barr, strep and Moon all negative, her sister had no fever and no cough, everything lasted 4 days then back to normal. XXXXXXX got sick 1 day after her sister got well.

We did not think they were realted as the symptom were not the same. No one else is sick and everyone got the flu shot.

her fever is 102.6

my mom says as long as she has those white spots in the back of her throat the fever will persist and will be high - moms know everything she is 84

we are doing warm water and salt to try to dislodge the spots

Possibilities -
1. Prolonged viral illness.
2. Streptococcal pharyngitis
3. Ebstein Barr viral infection.

Suggestions -
1. Do a rapid streptococcal antigen testing.
2. Blood culture.
3. EBv - VCA - IgM Elisa in blood
4. Throat swab for culture.

Treatment -
1. Warm saline gargling.
2. Add Azithromycin 10mg/kg/day once daily for 5 day to cover atypical infections.

Kindly get back to me once the blood tests are ready (if your doctor agrees t do them).
