Question: Hi Doctor.

Need an opinion on attached papers for Dementia and central visual field defects .By profession the patient is pilot and he has been put temporarily down from flying duties due to Dementia and central visual field defects.Need to know the detailed opinion on the ailment sustained by the patient.Also please confirm if the ailment is curable or not along with confirmation if its pre existing.

I am Dr.Ajay Panwar,a neurologist, here to answer your query.

I have gone through the reports attached here. The documents make it clear that he has dementia especially pertaining to frontal and parietal lobe domains involving attention, executive and visuo-spatial functions along with visual field defects. Clearly the job of a pilot needs good functioning of all the above mentioned impaired functions otherwise he may run into an accident.

Further, the diagnosis so far mentioned is just 'Dementia' with no definite cause of dementia mentioned.Cure or treatment depends on the cause.Some of the causes may be reversible like Hypothyroidism, B12 deficiency , Hydrocephalus(brain MRI does not mention Hydrocephalus though). However, most of the causes have no definite cure but improvement in symptoms with medications may occur.

To summarize, treatment and prognosis depends on the cause.
