Suggest Treatment For Cough At Night And Cold In Child

Question: My 2 and half yr old niece is suffereing form cold and cough. Her cough problem is from last 2 months. She mostly coughs while sleeping in the night. During the day, She does not cough too much as compared to night while sleeping. Her throats gets chocked while cough. It seems that something has stuck inside her that is not coming out. I think it's the sputum that is causing the problem. Sometimes when she coughs a very small amount of an off white color secretion comes out. This off white secretion is also coming out from her nose since last 10-14 days. We visited to Pediatrician + Pulmonologist. After diagnosing her, Doctor prescribed the following medicine
Ascoril LS+ (For 5 days- 3 times in a day), Montair LC (for 1 month- once in a day) , Iron (for 1 month- once in a day) and Calcium syrup (for 1 month- once in a day).
He adviced to stop taking Ascoril LS+ after 5 days. Doctor also said that if the kid does get any relief with these medication after one month then we will do the Asthma and Tuberculosis test to find out the root cause of cough.
After taking the Ascoril LS+ for 5 days,she did not get any relief from Cough. So Today i talked to the doctor for the same. He suggested us to give her nebulisation with budecort and Levolin for 5 days. He also said that if she did not get any cough and cold relief then go for the test of asthma and tuberculosis after 5 days.
Doctor can you help us to make sure that whether the budecort and levolin are safe to give 2 and half yr kid. Is there any other alternative of budecort ? Today i am surfing on google for the same. I found one medicine named "Mucomix". Out of these two medicine Budecort and Mucomix which one is best and safe. If there is any other alternative please suggest. Do we need to go for Asthma or tuberculosis test??

By what you quote I feel what your kid could be having viral associated wheeze or multi triggered wheeze. I have a few questions for you -
Questions:
1. How many days per month does she cough or feel breathless?
2. How many nights per month does her sleep get disturbed due to above symptoms?
3. Does she feel breathless when she runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?
If your answer is yes for any of the above questions, your kid might be having viral associated wheezing or multi triggered wheezing and I suggest you meet a paediatric pulmonologist who is near your place. No point in using antibiotics every time. It will do more harm than good by increasing bacterial resistance.

Hello doctor
Please find below the answer of your questions
1.she suffers from 5-7 days per month. But she does not feel breathless
2.5-7 days per month. And for this month14 days
3.she does not feel breathless while running or playing with the kids
4. We have visited 2 peadriatician in the past. According to them kid is suffering from these problems due to weather changes
5. Yes her mother is suffering from skin allergy (psoriasis). Her mother also go through continuous sneezing for 10-15 min. after taking bath.
6. Yes doctor her cough always associated with fever. But from last two months she get the fever only for one time. Now she does not have fever.
We already visited to a pulmonologist. He prescribed the ascoril Ls+ and montair lc . but she did not get relief. So today pulmonologist prescribed budecort and levolin nebulization. He said if she does not get relief with these medicine then go for asthma and tuberculosis test. Doctor please suggest are these medicine safe to give 2 and half yr child? Is there any other alternative of these medicine?

Inhalers are better than Nebulization

Suggestions:

1. There are certainly alternative management therapies in allopathy now-a-days. Medicine has advanced a lot and wheezing is 100% controllable.

2. Inhalers are the newest management strategies for this. If I were your paediatrician I would have suggested the use of Budecort metered dose inhaler (100mcg) 2 puffs twice a day through a spacer and this is for regular use for 8 weeks. Another metered dose inhaler is Levolin and this can used as rescue therapy whenever the kid is having severe cough in spite of regular usage of Budecort. The technique is very important and very crucial for the drug to be delivered correctly to the lungs. Regularity of medicine usage also matters a lot. So do not discontinue abruptly after you notice some improvement. The technique of administering an inhaler using a spacer has to be taught to you by your doctor and these are prescription medicines. So I suggest you consult your paediatrician for this.

3. Triggers can be environmental changes/ dust/ talcum powder/ seasonal changes/ un-cleaned a/c vents/ cold weather etc....we can specifically say this is the cause - unless we observe the kid closely - best person is the parent.
