What Can Be Done When A TMT Stress Test Says 'positive For Inducible Ischemia'?

Question: Dear Sir.. I have applied for a term life insurance and undergone TMT stress test for the same. My TMT is coming as abnormal and TMT is positive for inducible ischemia. My age is 35 years. Height 175 Cms. Weight 74 KG.Nonsmoker..I'm not diabetic. What should I do now? Should I go for Cardiac Angiography? My cardiologist has advised me to go for Cardiac Angiography? Regards XXXX

Understand your concern
TMT test is positive for inducible ischemia means:

Insufficient blood supply to heart due to increased work load on heart. It is due to narrowing of blood vessels of heart (coronary artery disease).
During resting stage, heart get sufficient blood supply.
You have to consult the cardiologist to investigate coronary artery disease.
You have to undergo for following investigations:

1. TMT with thallium isotope: It is non invasive test to assess ischemia/ physiological function of the heart.

2. CT angiography: It is non invasive test and assess the anatomical structure of the heart.

If above tests will give positive result then you have to go for coronary angiography and catheterisation. (It is invasive and gold standard test)

Dear Sir... Thank you very much for your reply.. I hope that you have seen my TMT report attached with my previous query..Sir..please don't mind.. I had already got the information provided by you through Google... Dear Sir.. I belongs to XXXXXXX armed forces and our service cardiologist is insisting to go for Cardiac Angiography (Dye test) on Saturday, directly to know the exact condition of heart... He has not insisted TMT with Thelium test... He said that if something is wrong it will be visible in Angiography... Sir.. I want to know that whether my heart problems is so serious.. And what are the chances of enjoying happy and successful family life.. And sexully acttiveness too.. Whether it will effect my sexual family life ... How much physical stress I can take now... Can I ran 4 to 5 kms normally.. Regards

Yes you can go for cardiac angiography directly

Understand your concern.
TMT test is not very accurate test to detect coronary artery disease.
Cardiac angiography directly visualize the block (if present), its location, extent and severity of block.
I already told cardiac angiography is gold standard and invasive.
If you have no problem with invasive procedure then you have to go for cardiac angiography directly.
Seriousness of heart disease and other query will depend on the result of cardiac angiography. So follow your cardiologist advice.

Dear Sir.. Thank you very much for your prompt reply.. I had a doubt in my mind that should I again go for TMT stress test before Cardiac Angiography just to check that might be this time it can show negative TMT. Because it might depends on machine to machine.. Is it going to help Sir? Regards

Look dear, TMT test is not very specific and sensitive test to diagnose coronary artery disease.
Do not go again for TMT test.
Best is cardiac angiography to rule out coronary artery disease for the TMT positive result.
