Suggest Treatment For Constipation And Anal Fissure In Children

default
Posted on Mon, 6 Apr 2015
Question: Hi Doctor,
My 4 yrs old girl has had constipation problem since she was 1 yr. At about 2 yrs she avoided going to pass motions and would scream n cry. Doctor said she had anal fissure and this followed with several agonising moments as we often had to use glycerine enema to make her poo. We are doing treatment with stool softeners n switched to fruit n veggy diet. She has improved over time but though she passes stools daily, the fissure is still there as reported by doctor in last visit as she sometimes complains of pain during bowels. We are giving her ayurvedic stool softener Softovac. Is there any further treatment we can follow to permanently heal the fissure. The doctor says continue stool softener and good diet habits as thts the only way.

Brief Answer:
Hydrocortizone/lidocaine ointment locally...

Detailed Answer:
Hi,

I am sorry for the situation your 4 years old girl is going through.

1. It is crucial to get her constipation problems relieved first as anal fissure will get improved once constipation is relieved too.

2. Your daughter is too young to use fissure ointment as in adults. Stool softeners and good diet habits will help her alot; but, the results are gradually noticed.

3. To ease her anal pain when passing bowel movement, I advise to use a combination ointment containing hydrocortisone and lidocaine (externally).

However, I still insist to continue following her doctor's advice in applying good diet habits and taking stool softeners on regular basis.

Hope it was of help!

Dear doctor...thnx for ur reply.
Kindly suggest a good diet plan for her which can be followed daily basis.
In the morning she takes a glass of fresh milk and goes to school. She carries sweetcorn or fruits in her tiffin box.
For lunch its basmati rice with fried fish and yoghurt.
Dinner is mostly soup containing spinach, sweetcorn, mushroom, brocolli. Boiled beetroots and sometimes Chapatis (brown flour).
We avoid giving her poultry products and meats except chicken.
We are hopeful with a good diet plan she will definitely show improvement.

Brief Answer:
try to avoid rice/yogurt; pasta and baked vegetables/fish instead

Detailed Answer:
Hi back,

I was reviewing your daughter's diet, and there are a few things I need to point out:

- rice is to get shorten as it will aggravate constipation. Instead, can ask her to have pasta or baked fish or vegetables

- yogurt is also to get shorten

- everything else is ok as per your traditional foods.
