Suggest Treatment For Constant Fever While Having Typhoid

Question:
Hello Doctor

I was having fever for last 8 days , after five days of fever I got my blood tested and they found its typhoid
now I am taking its medicines from last two days but no significant changes in my condition , still getting fever like symptoms , and from inside I am not feeling 100% well
my doctor said it will be over in three day but I have to take medicines for five days

usually how many days it takes to cure typhoid ?
is there any chance that it cant be cure by these antibiotics ?
I am taking all precautions , like lot of water , lite and clean food etc
Is there any chances that I will be worst affected because I had this deices before 12 years ?

Thanks for posting in HCM.
Typhoid fever is a water borne disease where the salmonella enters body through contaminated water and enters blood through intestinal walls.
Whenever Widal test comes positive we start patient with cephalosporin group of antibiotics which has been rightly started in your case.
But its an antibiotic whose effect after taking starts being seen after 3 days of starting so as you said its been 2 days you started the meds. Total course of treatment will be for 1 week to 10days.
You will surely start seeing the effect and decrease in fever and other symptoms after 3 to 4 days of taking antibiotics.
Total recovery takes about 10 days when you have properly completed the antibiotic course as prescribed.
Good that you are taking proper precautions like eating bland food, drinking clean boiled water and so on.
I would like to add here that as you told you had this 2nd time so it will be better to recheck your drinking water source and rectify the cleanliness of it. Because if you have got it even your family members are exposed to it and can get it. Rectify your drinking water ways and make it safe and clean.
Do not worry about getting healed. You are on appropriate treatment for typhoid and you will see the results soon.
One more thing is that as typhoid starts getting better the frequency of headache might increase. Dont worry its a normal course of the disease. Do not get alarmed. Take calpol or ibuprofen at that time whenever you get headaches.

I am living in XXXXXXX with my Brother , today he also got the typhoid positive in blood test .

I don't know we all drinking canned water at home and in office too , I had never drunk outside water , though eating snacks outside in evening , that may be the cause , in my locality in XXXXXXX its common now a days , lot of people are at doctors clinic

Can you please explain me these tests , I have type O and H positive but all other negative , is it mean that its initial stage of deices

one more thing .

is this virus get developed in the water automatically like we have common water tank in our building , or the water should be infected by an external source ?

we cant stop using this water suddenly or change our residence , can we mix some kind of liquid in it before using it for bath or brushing

Salmonella is not a virus, it is a bacteria. It is usually spread by feco-oral route, which means, if the water you drink, or the food/anything you eat has been contaminated by these bacteria (for water, it is usually by the contamination through a sewage leaking into the drinking water pipes, and for food, it is usually by the infected person not washing hands properly after passing motion, and touching the food).

The tests done are for specific antigens found in typhoid bacterium which is detected by doing WIDAL test. The titres if equal to or above 1:160 then it is considered positive and treatment is initiated. AH, BH and other are just other strains of this bacteria to point out the exact causative agent.

And even after you get treated completely and repeat WIDAL test after few days it might be positive as it takes time to get rid of the antigens totally but it doesn't mean that you are infected. In this test, patient’s blood is added to a suspension which contains specific type of Salmonella bacteria, so if the patient has any antibodies developed (which usually takes about 7 to 14 days after infection), then the antibodies from the patient will combine with the antigens in the suspension and it will become positive. But since this is an antibody test, the results always do not mean you are currently infected with Typhoid. If there is a 4 fold increase in the WIDAL titres during the course of the illness, then it is considered as a definitive indication of current infection with Salmonella.

Coming to your second question, typhoid is caused by bacteria of salmonella species. It will affect and cause typhoid only and only when it reaches the intestines when consumed for drinking purpose. It won't spread through water used for bathing and other purpose. I might advise you to check your caned drinking water source and change the source if possible. Otherwise consume only boiled water.
And more importantly, stop eating from outside, as it is difficult to know if the person who handles your food is infected or not.

Boil water until it is boiling well, then you can cool it and use for consumption. This precaution along with proper hand washing with soap and water before each meal will suffice to protect you from typhoid.
