Suggest Treatment For Constant Fever In A Child

Question: My child is 17 months old.He is having high grade fever about 104 F plus from 2days .He is also having mild productive cough and common cold. 4 days before fever,he has developed small macules on the forehead and on the lower back .After fever,these rashes becomes red and now spread all over the body but are more on face and hands as compared to other sites. When fever is high ,sometimes vomiting occurs.sometimes vomiting also occurs during coughing and phelgm comes out and then vomitng stops.Paediatrician prescribed microcef 50mg (5ml)×BD.Crocin for fever.but fever never goes down . Now we are giving paracetamol 125mg+ mefenamic acid 50mg (5ml at 8 hours interval).fever respond well to it.Paed also prescribed a cough syrup and an anti allergic (levocetrizine)1/4 tab once a day. Is treatment is right for my child.I also want to know whether it's safe to give this combination to subside the fever. Please help me.

I have gone carefully through the details and understand your concern. Most often such symptoms are due to viral infections and antibiotics are ineffective in such cases. Irrespective of use of antibiotics, it usually spontaneously resolves in 5-7 days. However, since you have started the antibiotic, please complete the course.

If your child has fever with temperature more than 100°F, you need to give paracetamol (P-250, Crocin or Tylenol) to it bring down. If paracetamol is not effective in bringing down the temperature, you need to use Meftal-P (mefenamic acid) which is more effective. The two need not be simultaneously used. It is safe however, assuming that they are being given in correct dose. Also use tepid sponging. Giving steam inhalation to your child helps in reducing cough. Water vapor will reach deep inside and would help in diluting the cough and thus would provide relief.

Levocetirizine is an antiallergic and should take care of the allergic symptoms which are often associated. Nothing else needs to be done.
