Suggest Treatment For Bladder Infection Caused After Vaginal Hysterectomy

Question: Hi there! I had surgery April 29, 2014. I had a vaginal hysterectomy, anterior work and a bladder tack with a sling. I did fine in the hospital but then they took the catheter out, I drank water, I walked the halls and I could not void. I begged for them to put the catheter back in after just a little while - I was filled to 800 cc's. It hurt. I went home with a catheter and antibiotics - cipro. I did fine at home. I saw the nurse at the urologist exactly a week later. She removed my catheter around 10:00AM. She told me to go home and if I voided that was great. I did void and I thought all was fine until around 9 :00 PM I got that awful full feeling - tons of pressure and my sweet husband rushed me to the ER. I had almost 900 cc's in my bladder. I had a raging UTI. I had just finished the cipro that day. They gave me a shot of Rocephin and sent me home with the catheter and Augmenten. They cultured my urine. I will make a very long story short - I had another ER visit the next weekend even with the catheter. I started another antibiotic and they did another culture. I ended up having the catheter for almost 3 weeks. I have been self cathing for 10 days. I had a call this Tuesday late afternoon from my wonderful gynecologist. After my post op visit to him last Friday I still had an infection. He wanted to start another round of antibiotics because my culture at my post op appointment still showed an infection. In the meantime I have yet to be completely comfortable! I am getting discouraged. I went to see my GP the very next morning and I said help!!! His office has given me a shot of rocephin for three straight days. I feel better. I still feel like I have to urinate all of the time. When I XXXXXXX nothing but a little residual is there. But I still feel like I have a constant pinch between my legs and the minute I leave the potty I feel like I have to go again. I just want to know is this sort of normal. I worry that my sling is too tight. My urologist says that I am fine and it will all work out. He said rarely does he loosen them but he can. I am very active - I play tennis, walk, run, ride horses. The doctor said that I had almost no bladder support left. My two precious children were large so I am glad I did it but I just want hear from someone that it will all work out!!! I can take pain, I can XXXXXXX but I do not want to feel like this forever!! Thanks so much for your help!! It feels good to let this all out!! Thank you!!

Brief Answer:
Some suggestions.

Detailed Answer:
Hi XXXX
Thanks for consulting us.
I agree that you are facing a depressing situation, but, such complications can occur after these surgeries.
See, your urine culture tests are coming back positive. this mean an ongoing or recurrent infection. This also give us hope that all will settle to normal once this infection is under control.
This is good that you have learned the self catheterization technique, it will help you a lot.
I have one suggestion for you. You may discuss with your doctor about doing an ultrasound or X-ray KUB ( Kidney , ureter, bladder). This will tell us about any obstruction in the urinary tract ( like a tight tape as thought by you) & any other pathology which is causing this problem apart from infection.
You may sort a second opinion from a urologist, if no success.
Hope you get well soon.
Best regards.

Thank you so much! This helps a lot. Is there a time frame when it is better to go back in to alter the sling? Should we go back in sooner than later? Also, we have a graduation party tonight. I have not been out much and would it be okay if I had a glass of red wine? Would that affect the Rocephin? It is not big deal if I cannot but if I can I think I might. I am not taking pain meds anymore. Thanks so much!

Brief Answer:
Wait for 6 weeks to let this settle.

Detailed Answer:
Hi,
Welcome back,
No, I don't think it would do any harm, as long as you keep the bladder empty. As retention can lead to back pressure & hydronephrosis which can damage your kidney.
I think you should seek second opinion from a urologist if the problem is still there after 6 weeks post operation.
I don't feel red wine will effect your Rocephin's action.
Just be careful do not develop retention & don't let yourself harmed.
Best wishes.
