Suggest Treatment For Anal Fissure Causing An Abscess

Question: 1. In Last week of July-2015
Actually, I also had antibiotic 5 days course for my Digestion Problems along with probiotics for 4-5 days.

2.In 1st week of SEPTEMBER-2015,
ENT doctor prescribed ADVENT (anti-biotic) for my Throat Infection for 5 days
BUT WITHOUT any pro-biotics.
While i was taking PAN-D early morning empty stomach with those Anti-Biotics.

3. At the end of the course on 6-SEPT-2015 -
after hard antibiotics, my stomach is feeling disturbed. I FOUND LITTLE BLOOD IN MY STOOL AND LITTLE PAIN IN MY ANUS/rectum portion

while passing stool and i had food without spice since then.
Along with this in these couple of days i feel like swings in my lower abdomen before passing stool.
I used RootiCool gel to be applied in the rectum/anus on my own.
along with Stool softner.

4. As regard to my anal fissure, i had been taking less spicy food.
--- After 4-5 days i felt Okay. and took normal home food for 2 days


5. Now, in since, 1st OCTOBER-2015 - The burning sensation Again Started in anal portion while passing stool.

6. AND NOW on 06-OCT-2015
I again started taking food WITHOUT SPICE.
And feeling okay while passing stool since yesterday

7. BUT Just half-inch above the anal i am feeling some corn which is painful and made difficult to sit me properly. The corn is half-inch above anal and

painful when touched from outside.

--- Today morning i felt that corn is more inside than outside.

8. Can it be some side effect of my anal fissure? i am feared.

Kindly explain the symptoms with your opinion.

Thanks for your query.
Read and understood your history and concerned.
To summarize, antibiotics disturbed your GI flora, there was little blood and pain in the anus/rectum are. You started with food without spices, applied RootiCool, took stool softener - feel like swings in lower abdomen before passing stool - since 1 st Oct burning at stool - Now just half-inch above the anal you are feeling some corn which is painful and made difficult to sit properly. The corn is half-inch above anal and painful when touched from outside. Today morning you felt that corn is more inside than outside.

Yes it may be due to anal fissure producing an abscess, the infection spread from the fissure, as fissure is a wound related to the anus and the stool contains very high concentration of bacteria.

Your concerns are as follows and will explain as you have asked:
*Precautions:
Do sitz bath- means sitting in a tub with warm water
Do not press on it nor sit for long duration
Get examined by a General Surgeon by digital examination if the pain allows.
It is possible that your Surgeon may ask for MRI study, may not ask and go by the clinical judgment.
He shall also prescribe you proper antibiotics, metronidazole, stool softeners, Pre and probiotics, anti-inflammatory and whichever is his choice. You need a prescription.

I am Thankful by heart for such a detailed answer.

I have been taking food totally simple without spices.

My meals are being cooked only with salt now a days along with I have
been taking Milk, curd sometimes.

In the beginning the doctor prescribed sitz bath with "potasium pomagrate" - spellings mistake.

1. Kindly tell sitz bath how many times should i repeat.?

2. Sat-Isbagol - i.e. Psyllium Husk - Can i take this for fibre need.?

3. Are these the sypmtos of "INTERNAL HAEMOROIDS"...?

4. Sir, you have explained beautifully. I am very afraid of getting
operated for the same. Should i share some picture of the infected area.?

5. Can you please suggest stool softners. I will by highly thankful to you.

I feel, it will very painful, if physically examined. Let's see whats on the cards.

How much are the chance of getting it cured as per symptoms, Without surgery.

I will try to answer your questions point by point:

1. Kindly tell sitz bath how many times should i repeat.?
Sitz bath should be taken at least twice daily in the morning- once before you pass motion and once after you pass motion to keep the area soothe and to clean the area properly. No need to add anything to the water. Potassium permanganate is added to make the water sterile. You can add enough to make the solution pink (not darker).

2. Sat-Isbagol - i.e. Psyllium Husk - Can i take this for fibre need.?
If your motions are hard you can certainly take Sat Isabgoal 1 to 2 tsf in a glass of warm water in the night. This helps to soften the stool as well as add fibers.

3. Are these the sypmtos of "INTERNAL HAEMOROIDS"...?
No this is not look to be due to internal hemorrhoids. They are diagnosed only on proctoscopy examination.

I can understand, everyone is afraid of surgery; but if there is no alternative one has to undergo surgery.

Isabgol husk is the best. Try this first and only if it does not work, we can add something like Cremaffin plus 1 to 2 tsf at bedtime

I feel, it will very painful, if physically examined. Let's see whats on the cards.
Your Surgeon will not check further if there is pain. He may then advise you to go for MRI study.

6. One more question Sir,
How much are the chance of getting it cured as per symptoms, Without surgery.
If this is a small thing, it may resolve with proper medicines.

7. How much time a surgery, if happens, would take me to recover.
The time to recover will depend upon the proper diagnosis as I have mentioned in the first answer. Hence is the need to go for a perfect diagnosis.

Respected Sir,

I am again highly obliged with such professional help.

1. I have sought your professional expert opinion,
since because i have been taking homeopathy for the last 5-6 days.
And i was afraid of being getting operated through surgical methods.

That's why i have been trying my best to control this in a natural way.

2. Little change in my symptoms,
From 2-3 days i have been taking
- Curd
- Chaaj
- Salted chapati with salid
- Sat-Isabgol once a day
- Milk at night.
- Sometimes apple peeled.
- stool softner DUPALAC

Even when I have eliminated 100% spices from my food, I found that while passing stool its burning sensation, although stool softener work somehow.
Soon after passing stool i use to take sitz bath immediately in very less warm
water in a tub.
And in about 30 minutes pain relieved to 80%.

3. Along with that ---

THE PICTURE I AM SENDING TO YOU IS ONLY TO EXPLAIN THE PORTION
WHICH IS PAINFUL FROM OUTSIDE WHEN TOUCHED.
(marked in the picture)...... IT IS LYING swelled 1cm ABOVE THE ANUS.

AND WHILE SITTING ON ENGLISH SEAT WHEN I TOUCH THAT PORTION
IT IS LIKE SWELLED VEIN SOFT TO TOUCH AND PAINFUL.

SIR, I AM MORE WORRIED ABOUT THAT PORTION,
Its more painful.

It is requested to provide your valuable opinion.

4. I have had taken Sporlac-DS(mouth dissolving pro-biotic)
for 3-4 days, few days ago.
CAN I TAKE this to support my GUT.

Sincere thanks for your appreciation.
I can not really say about homeopathy as I do not know what medicines have you been give.
You definitely need an antibiotic, metronidazole, anti-inflammatory and an opinion of a General Surgeon.Timely proper diagnosis and treatment can alleviate Surgery in some cases, but if you get late the complications go on continuing.
In spite of restriction you still have burning, try to take normal full diet as it helps to form good stool and continue the medicines you have mentioned including Sporalac DS.

Feeling LIKE SWELLED VEIN SOFT TO TOUCH AND PAINFUL is indicative of an abscess formation, this may open up on its own or may need surgical drainage after confirmation of a General Surgeon; please consult one and let me know what was the progress.
