Brief Answer:
Comprehensive treatment is needed.

Detailed Answer:
Hello,
Thanks for trusting healthcaremagic!
I have gone through your query and I can understand your concerns.

As per your description it seems that you are having diabetes mellitus along with multiple comorbidities. It is quite common among middle aged people who follow a sedentary lifestyle. You need comprehensive treatment and life style modifications to overcome this.
I suggest you to consult a general physician and get the following thing s done:-
(1)- The most important thing is your blood sugar level. You have to keep your blood sugar level with in normal range. Metformin is very effective in diabetes patients with associated obesity. It helps to lose extra weight. Do your FBS(fasting blood sugar) to assess the progression.
(2)- Avoid oily and fatty food. It helps to lose extra weight and also helps to reduce acid reflex.
(3)- Keep your self engaged. Do physical exercise on a regular basis. Swimming is very effective to lose weight. If it is not possible do jogging or indoor sports like badminton.
(4)- You are having acid reflux due to your hiatal hernia. You might need drugs like omeprazole to control acid reflux and heartburn. Avoid spicy foods and alcohol.
(5)- Always have smooth and fiber rich food to avoid IBS related symptoms.
(6)- Do frequent visits to your physician atleast once in every two months.

Hope i have answered your query.
Wishing you good health.
Regards.
