Posted on Tue, 7 Jun 2016
Question: I'm having pain in the upper, right abdomen. I took milk of magnesia last night due to constipation and had a pretty thorough evacuation this morning. The pain is persisting and I have this pain before. Should I be concerned?

Detailed Answer:
Hello and Welcome

I appreciate your concern

Right upper quadrant pain is commonly related to Gastrointestinal causes such as GERD/ gastritis, cholecystitis. It can also be due to a kidney stone especially if the flank or back is involved.

However, if the pain is progressively worsening then a cardiac cause should always be ruled out first. An EKG is usually done just to be sure. If its clear then an ultrasound of the abdomen specifically looking at the biliary tract including the gall bladder should be advised. The ultrasound will also show if there is an calculus in the urinary tract. If none are found then GERD/gastritis should be treated empirically with PPIs like lansoprazole and pro kinetics like itopride. Antispasmodics can be given for the associated pain.
