Suggest Remedy For Loss Of Bladder Control After An Injury

Posted on Mon, 23 Mar 2015
Question: four days ago I took a bad fall onto a concrete sidewalk landing my left side. I hit the concrete so hard that it felt like my bladder exploded and have massive pain and loss of bladder control. I had a bladder ultrasound done yesterday, they told me that there was an unknown structure measuring 3.9 x 1.6 cm outside the bladder. but today I had a cystoscopy and they said the bladder was ok inside. so, next week they are doing a CT scan of my abdomen and pelvis. BUT I had no pain at all or loss of control of bladder or massive pain UNTIL I FELL. do you think this is a mass such as cancer or could it be something related to the fall? also for about three years, I have severe chronic constipation and have had several colonoscopies done and they always snip out polyps. but I kept telling him it feels like there is something pushing inward toward the rectum. he said I just think that is the way it feels an nothing is there. but there is something there and I fired him. please let me know what you think. I would appreciate it. thanks. XXXXXXX
thank you XXXXXXX

Brief Answer:
Loss of bladder control related to fall

Detailed Answer:
Hello,

I have gone through your query and I understand your concerns about structure outside bladder .

Following is my reply:

1. The structure seen on ultrasound scan could be due to Blood collection after fall.

2. In view of repeated polyps in colon, it is likely that they are also due to cancerous lesion. But based on the history alone it is difficult to decide and CT scan is needed to comment further.

3. Loss of bladder control is not related to this structure as loss of bladder control started only after fall. It is more like to be injury to urethra or bladder neck sphincter.

4. Hence I presume this is more likely to be related to fall and cancer chances are low though it exists.

I hope your doubt is clarified. Let me know the CT scan report so that i can advice you further.

so you are possibly saying that maybe this is not cancer? it could be related to the hard fall to the ground? I am due to have a CT of the pelvis and abdomen in three more days. is it possible this this 3.9 x 1.6 cm structure is perhaps not a mass or cancerous? This gives me hope and I am wondering why is the bladder AFTER THE FALL so hideous pain and feel swelling and loss of bladder control. Could this blood collection JUST APPEAR TO BE A LESION on the Ultrasound?? what can I do to ease the pain until the CT is done next week? pain pills are bad because of my chronic constipation. Would Ice and Heat help? Please give me advice. I will send you a copy of the CT report next week. thank you and will wait for your response.

Brief Answer:
Yes. This is likely to be related to fall

Detailed Answer:
Hi,

You are right.

I think this is likely to be related to hard fall on ground and not cancer.

The structure could be blood clot ( hematoma) due to fall. This appears as a lesion in ultrasound Difficult to say without a CT scan.

Yes loss of bladder control could be related to fall as it could have damaged sphincter.

Since injury is severe and deep inside, you need painkillers. Ice or heat is unlikely to work in this.

We shall review it after your CT scan report.

Let me know if you need anymore help.

Hello

Dr. XXXXXXX

Here is the copy of the bladder ultrasound report, which I was sent to my urologist as the ultrasound showed an unknown structure. And they said I needed a CT scan of abdomen and pelvis to determine what this unknown structure was. as you know, i fell on the sidewalk very hard on my left side on 2-20-2015 and felt like my bladder exploded. and horrible pelvis pain on left side. I am attaching the copys of these two documents. they found nothing on the CT scan of abdomen and pelvis three days ago. it did not see this unknown structure. the urologist called me and said it would have to be OPERATOR ERROR. i asked for another ultrasound of the bladder to see if its gone, he said no that he was sure it was operator error, but i feel that it still could have been a big hematoma they found on the bladder ultrasound as i fell down so hard on my pelvis and bladder area. they did a cystoscopy and said the bladder looked good inside. can you let me know your kind opinion. the left groin area is so weak and trying to walk is make me so sore. both left pelvis and left groin are massive pain. thank you

Brief Answer:
Resolved hematoma

Detailed Answer:
Hi

Welcome back. It's good see that structure doesn't exist anymore.

I saw your report.

As discussed earlier, that structure was a hematoma due to fall which would have disappeared soon as it gets absorbed.

Either ways it is good to know the structure does not exist anymore (either operator error or not).

Hence I suggest you continue your present treatment schedule to correct the damage that has occurred from fall.

Pain in groin and pelvis are due to injury after fall and take some more time to heal.

Please continue with your doctor's advice.

Let me know if you need any more clarifications.

Regards
Dr. Soumya
