Suggest Remedy For Eye Pain, Floaters And Pressure In Eye While Diagnosed With Polar Cataract

Question: My wife aged 49, has been diagnosed with polar cataract in RE. Further she is undergoing treatment for Eye Pressure (eye drops for last 18 months) as Glucouma is ruled out. Even during sleep and after waking up she feels pain the RE. She sees white flashes in both eyes and some floating threads too. I am attaching the latest prescription and snaps. Please advise the best action forward. After her root canal treatment in her lower jaw she experienced more vision problem including eye pain. In case cataract operation is advised, will it rule out other symptoms ? Which surgery is most advance available in Delhi/NCR and who is the best surgeon. Give the indicative budget for such operation.

Dear Doctor,

You can see the attached report and prescription by PERFECT WELLNESS CLINIC.

Initially doctor suspected Glaucoma as pressure was high, but after several photographs of the eyes and "Field Analysis" (All reports attached), it was concluded that there is no symptom of that, eyes are healthy.

So she is NOT a patient of Glaucoma...but the exact cause of high pressure is yet a suspense.

But pressure has to be kept under limits. For that she prescribed Eye Drops "Xavatra T" and "Osmodrops"She takes BP medicines..."METOSARTIN 50" daily
She is having Homoeopath treatment for Gynae problems and Vasculitis.

As prescribed by Gynae and Skin specialist - From March to September she was put on steroid medicines "MODUS 10 mg", Zaxid and Psorid. Currently none of the above medicines as she has switched over to homoeopathy.

I can certainly understand your concerns.

I apologize for the delayed response owing to my work schedule.

I have worked through your attached images in detail.

Your wife is rightly prescribed the medication to lower the eye pressure.

Her current symptoms could possibly explain vitreous degeneration which is common in individuals with high blood pressure.

It is very important to continue these prescribed medications, Rather resorting to homoeopathy. It does more harm than good.

Since the blood pressure and eye pressure well controlled and Glaucoma is reasonably ruled out based on fundys findings, she can go ahead with Cataract surgery.

The pituitary adenoma 0.3mm is no major a contraindication for surgery.

Dear Doctor,

My question remains unanswered...I wish it is reviewed by some Eye specialist.

You have suggested for cataract operation....but what about the white flashes in eyes and thread like floaters. Will above problems remain even after cataract operation?

Is Eye pressure shooting up because of steroid intake?

Homoeopathy is being considered for Gynae and Skin problem and not for eyes.

Hence please get my case reviewed by a XXXXXXX eye specialist. I am sorry to say that I am yet to be convinced by your answer.

Hello, Sir.

I apologize as my answer could not convince you.

The symptoms of white flashes and glaring threads are due to vitreous degeneration. Vitreous is the posterior segment(back part of eye) and lens is the Anterior segment (front part of eye).

In cataract surgery, only the lens is manipulated, but not the vitreous, hence those symptoms persist even after the surgery.

Cataract surgery will NOT lead to disappearance of white flashes and floating threads.

Her drug intake from March to September , Only MODUS 10 mg is a steroid, PSORID is an immunosuppressive agent and ZAXID is an antifibrinolytic agent which is known to reduce excess bleeding.

The later two drugs are no where associated with elevation of eye pressure.The only steroid is MODUS 10 which contains progesterone which is also not proven to cause elevated eye pressure in clinical trials.

Hence, she was prescribed these drugs based on her vasculitis and Gyne issues, considering the need and lower risk benefit ratio.

The cause of persistent elevation of eye pressure in the absence of field changes is due to a condition known as OCULAR HYPERTENSION. This is a forerunner of (future)GLAUCOMA.Hence OH should be viewed and treated as GLAUCOMA SUSPECT.

XOVATRA T is dual agent which is an excellent agent used in treatment of OH to prevent the development of GLAUCOMA.

To sum up, GLAUCOMA causes posterior segment pathology which has to dealt separately. And Cataract is an Anterior segment pathology which has to be dealt separately. Treating the one will NOT solve the other.

Likewise, Vitreous degeneration (white flashes and floating threads) require treatment with LASER PHOTOCOAGULATION or CRYOTHERAPY for cure.They will not vanish after cataract surgery.

To sum up, your wife has 3 distinct eye issues which require specific therapy.

1.Ocular hypertension (OH)-XOVATRA T
2.Polar Cataract-Surgery.
3.Vitreous degeneration-Laser PHOTOCOAGULATION.

Based on abovementioned, she can go ahead with cataract surgery with eye pressure maintained with in normal range with the help of XOVATRA T.

OSMODROPS only prevent the dryness of the eye ,but are NOT intended to lower eye pressure.
