Question: My husband has cirrhosis of the liver. He was taking ZZZ-Quill, over the counter, but it lost its efficacy recently. Is there anything he can take to help him sleep that is all right with his condition?

I understand your concern.

Sleeping pills are best to be avoided in cirrhosis of liver as they may some times precipitate abnormal behavior, altered sensorium and even coma.

These patients may have altered sleep pattern like they may sleep in day time and stay awake at night.

Also it depends on stage of this cirrhosis, if he has such history or has water in tummy (ascites) then use no sleeping pill at all.

If in early stage, can try some medication but with immense caution like zolpidem 2.5 mg but that too after consulting your gastroenterologist and psychiatrist.

can try following things too,

1) Ensure he has good sleeping pattern, avoid sleep at daytime.
2) Ensure adequate physical activity.
3) do not watch television or do activities which strain your brain as it may hamper in initiating sleep, can read a book or listen to soothing music at sleeping time.
4) ensure adequate bowel movements with laxative like lactulose to clear ammonia and related toxins from body as they may alter sleep rhythm.
