Suggest Diet And Lifestyle Changes For A Diabetic With BP

Question: Dear Doctor,
I brief below my present health condition along with my life style and food habit with a request you kindly advise on my diet and changes required thereon, if possible calories involved on the items which I am taking.

Brief Details:-

Diabetic (Type -II) - 19 Years
BP - 12 Years

• Vegetarian
• Non-smoker
• Non-alcoholic
• Age – 57 Years
• Weight 84 Kg
• Height 176 CM

My present problem: -

1) Morning Tiredness & uneasiness: - I normally feel tiredness & especially during morning
2) Now a days my weight is increasing day by day?
3) Though I want to decrease the coffee consumption, but some or other reason I could not, but
taking without sugar. Is there any considerable adverse effect on this coffee drinking habit?

Physical Activity: -
Moderate Walking – Not very regular due to tiredness

Nature of Work

Desk works with normal work related stress.

Family history
1)     Father – Died due to heart attack & No history of diabetics
2)     Mother – Died due to sun stroke during her holy travel & no history of diabetics & there was a
BP at the time her death.     
3)     Brothers (two nos) – No BP or diabetics
4)     Sisters (4 nos ) -- Two elder sisters having diabetic after their age of 60 & above

Present medications:

Insulin: MIXTARD 30/70          26-0-16
Tab: TRIGLYNASE      2mg            1-0-1
Tab: TELMED AH                 1-0-0
Tab: FOLVITE                      1-0-0
Tab: RESTYL     0.25mg            0-0-1
Tab: TELMED AM                 0-0-1

You need to get your HbA1c and thyroid profile

Detailed Answer:
Dear Sir, greetings from HCM . I understand your problem s.

First you need to get your thyroid hormone profile, HbA1c, cortisol, Hb%, microalbuminuria and lipid profile done. Your medications are OK. I have gone through your diet chart, its very good . but too much coffee even without sugar is not good. Try to stay away from it for a few days. When you become used to living without it, you can stay away from it for longer periods.

You can add millets , boiled or in the form of breads to that. That will give you more of micro nutrients (vitamin and minerals) , which are deficient in patients with long standing diabetes.
Even nuts, legumes, turmeric, garlic, oats all helps. Keep changing the type of vegetables and fruits all nutrients will not be present in a single source.

Morning tiredness might be due to micro nutrient deficiency/ thyroid hormone abnormalities, or even cortisol levels must be low, even anemia too.
Hence we need to investigate properly and then decide the line of management.

Yes your family history but that is not significant now as you already have diabetes. You need more physical activity again important to help insulin activity. Gaining weight due to insulin resistance . try out exercise and nutritious diet as I explained you or else you need a insulin senitizer gliptins or pioglitazone. Dear sir remember that lifestyle changes are very important in management of diabetes and hypertension. Hence they are called as lifestyle diseases. Try yoga, meditation, pranayama, music therapy .. All helps in bringing down your stress and related complications.

Sir, please advise on the followings:-
1) Enclosed my latest Annual Report _ My medicines are changed as above after this report and condition is slightly better now.
2) Thyroid profile _ Within normal limits.
3) Could you please have a look on the diet chart and is that RAJMA, Nuts & its quantity are causing to my weight gain or coffee ?
4) Yes sir, I do agree that, my physical activity needs a big improvement and I will try to put my best shortly
5) Can you explain about millets please

Dear sir, gone through your attachment s . things are ok . small amounts of nuts and rajma of good quality is quite good.
Milets are small nutritious grains , packed with minerals and proteins. eg. Bajra, ragi, barley , etc. See what is available in your living place.
Physical activity must.
