Question: Hi,
My question is related to alternative to Remeron for insomnia (without weight gain). Whether a ketogenic while on Remeron is problematic or ok.

I understand your concerns.

If you want to get off remeron and need an alternative drug to help you sleep, I would suggest you ROZEREM (Ramelteon).
It is a safe hypnotic with no risk of dependence.
You can talk to your doctor about this.

I do not think there is need to worry about ketosis while on remeron. I have never seen this in my experience.
Ketogenic diet shouldn't be problematic with remeron either.

On tapering Remeron:
One time I forgot taking Remeron it took me 15 days to get any reasonable sleep again. Now I have been tapering remeron using 1/2 a year to get from 30mg to about 18,75 mg (doing this very systematically by shaving of tiny amounts from the chewable tablets). Based on an understanding of the long term effects and changes in (I guess neurotransmitters):

What would be a reasonable time to get down to zero mg? (I am sure it is far quicker than what I have been doing)

Thanks for the suggestion of Ramelteon (I have now been trying to find out if it is approved in Norway).
Can Ramelteon be started while still on Ramelteon?

I can read that Ramelteon is a "melatonin 1 and 2 receptor agonist" - does taht means it mimics Melatonin as a neurotransmitter?

I hope this means it is more effectful than over the counter melatonin tablets? (have tried that)

Mirtazapine and Ramelteon can be taken together.

Sorry for late response.

Yes that's quite a long time to taper down mirtazapine. However, some people are highly sensitive to withdrawal effects.
So even if this very slow pace of tapering down is suiting you, you can continue with this irrespective of time taken to completely stop the drug.

Ramelteon is melatonin 1 and 2 receptor agonist. It mimics melatonin but is more effective hypnotic than melatonin tablets.
It can be taken along with mirtazapine.
