Question: My mother age is around 78 years old and fatty. She is diabetic since 15 years and hypertension also
Recently her suger level not decreasing. High blood suger. After food it is around 330 and fasting around 200.
She is taking medicine but not improve

Hello,

I have gone through your query and reviewed attached reports. Here is my advice.

As per attached reports, higher blood urea level, low creatinine clearance with leg swelling indicates nephopathy or kidney disease. So to know more about kidney function, I would suggest lab tests urine routine and albumin creatinine ratio.

In nephropathy cases, good glycemic control is required to reduce the progression of disease.

But in your case, Higher HbA1c level may be due to low hemoglobin level. So this may not clearly indicates glycemic/sugar control.

It seems that you have already showed these lab reports to your physician and he might adjusted anti diabetic medications as per her kidney function test i e creatinine clearance. Still in case of higher blood sugar level, I would suggest insulin injection Glargine after Diabetologist consultation.

I would suggest you to check blood sugar levels at least three times a day as there is a risk of hypoglycemia or low blood sugar due to reduced kidney function.

Now answer to second question, She should strictly follow diabetic and low protein diet i e avoid sprouts, dairy products. Fluid intake should be less than 1 litre. She can take green leafy vegetables and items of wheat/Sorgham floor with
upma(Rawa) in breakfast.

Today at home blood test carried out by glucometer
After Breakfast— PP IS 340
After LUNCH –PP IS 311
One of my relative Dr suggested to try following medicine after consulting diabetologist
1.     GLYCINORM –M 60 OD
2.     TENDIA
3.     BECOZINC
Earlier she is taking TRAZENTA 5 MG & DIAMICRON XR 60 MG
From today mother stops milk and curd .
We will carried out urine Routine & ACR .
QUERIES---
1.Whether she will continue the medicine TRAZENTA 5 MG & DIAMICRON XR 60 MG or change medicine . GLYCINORM –M 60 OD & TENDIA
2.At present she is not ready to take insulin .
3. At this age she can not do exercise and walk outside due present condition COVID 19 .
4. Can she take DAL Apple , Papaya, Pomegranates ,walnuts Almonds
5. She is also suffering with hiatus hernia so leafy vegetables comes out from mouth .

As per your history,

Tab. Trazenta (Linagliptin) and Tab. Tendia (Teneligliptin) are belongs to same group i e DPP 4 inhibitors and both are equally safe and effective in kidney disease.

Tab. Glycinorm M 60 OD contains Metformin addition to Gliclazide. Usually metformin is not advised in elderly patients having reduced creatinine clearance/kidney disease as there is high risk of hypoglycemia/low blood sugar level.

So if you have to continue metformin, strict blood sugar monitoring with periodic renal function tests are required. So before that kindly consult with diabetologist. So considering current sugar levels, I would suggest to start insulin injection.

In diet, She can take fruits apple, guave, papaya and vegetables bottle guard, Tendli, capsicum, cucumber instead of leafy vegetables. It would be better to avoid Dal.
