Question: My grand daughter ( 2 years) is having fever for almost 40 days on and off. As per physician advice we give Crocin syrup, then fever comes down . Then again shoot up. Blood test results indicates nothing . All parameters are normal , except hp which is about 7.8. While in fever she refuses food. Doctor says , no dengue or typhoid . Platelets count normal. But fever is recurring. What to do?

I have gone carefully through your query and understand your concern. The off and on fever seems concerning. I would insist on getting a blood CRP and procalcitonin levels checked (if not done already) and to share the reports. I would also insist on sending for a scrub typhus panel. I am not sure what you mean by hp. I would insist you to upload that report and also to upload the complete blood count details. If you have the details of temperature spikes, that can really be helpful. I would insist you to share it (if available). Also ask the treating physician whether the liver and spleen are normal or whether they are enlarged. Please share the details.

Thanks for your response. The haemoglobin is low and the cause of anaemia needs to be identified. Packed cell volume (PCV) is low indicating the likelihood of iron deficiency anaemia. Iron supplementation is likely to be needed.
