My Bladder Size Has Shrunk To 250 ML For Some

Question: My bladder size has shrunk to 250 mL for some unknown reason and my eyes keep getting unfocused every few minutes.

I am a 22M, and around 5-6 months ago, I started having some bladder problems. For the first month, I just ignored the problem and assumed it will simply go away, but it didn't and felt like it was getting worse. My symptoms are simply that I feel constant pressure in my lower abdomen, throughout the day, as if my bladder was just always full, but then when I went to the bathroom, not much would come out (if anything came out at all). This also made me go to the bathroom very frequently. I don't seem to have any symptoms other than this.

So I went to my general physician, who asked me to get a blood test done. The test showed that I had high WBC (12,200 Cells/cmm) and he suggested that I most likely have a infection (UTI) which is causing this. He took a while (a couple of weeks) before he prescribed me some antibiotic (Augmentin 625mg) to address this (we waited because I also had a ingrown nail issue, and he wanted to get that removed first for some reason). He also suggested that I get a ultrasound done, which I did and it came out normal.

The antibiotic didn't do anything, the problem persisted. We got another blood test done. This time my WBC count came out normal, so the tests really showed that everything was normal. He then asked me to get a Urine Culture done, and based on that report, he prescribed me another antibiotic (Nitrofurantoin). This one kind of felt like it helped a little bit, and it also changed the color of my urine to very dark yellow whenever I took it. But the problem still persisted. At this point, he suggested that I meet with a specialist. I procrastinated on this for a while (a few weeks) and then met with a Urologist.

The Urologist immediately checked if my penis to see if my dick hole was too tight, if my foreskin was pushing back properly or not, and we discovered that I was not clean under my foreskin. He suggested that I wash the area under my foreskin regularly, at least 2 times a day for some time, and to maintain my hygiene down there. He suggested this could be the cause for a UTI. He also prescribed me some medication (Zostum-O 200mg & Roliflo OD 2mg). I started cleaning myself down there daily, and the white line that was very clearly visible on the edge of the head of my dick went away after a while. I finished the meditation course, but nothing changed. The problem persisted.

I went back to the doctor, and he asked me to get a frequency chart prepared, so that we have a more clear understanding of how frequently I am going and how much urine I am passing. I did this, and we found that I am going to the bathroom about 15-25 times per day on average, and each time I am passing only 5 mL - XXXXXXX 250 mL, nothing more than that. He pointed out here that my bladder capacity seems to be very low, and suggested that we need to get a endoscopy done and see what's happening inside.

We recently go the endoscopy done, and found that my bladder size indeed seems to be very small (250 mL). I told him that it was completely normal until about 5-6 months ago, I never really had any problems. He also found that I did have some infection in my bladder. My report says that I have "diffuse eryhematous spots present in entire bladder, bladder capacity 250 ML" Based on this, the doctor gave me another medication (Megatas 25 mg) and asked me to take them for 1 month, and then get back to him. He said if this doesn't work, they might have to give me a Botox injection. They might try 1 or 2 other options, to try to expand my bladder. He suggested me to drink less water and try to hold my urine as much as possible. He couldn't really give any explanation for why my bladder size has shrunk in such a manner suddenly.

The doctor also tested my urine sample for Tuberculosis, but it came out negative. So we don't really have any clue what's actually going on, what kind of infection I have and why isn't it going away, and also why my bladder size has shrunk so drastically.

Other stuff to note, that might or might not be relevant to this problem :

My sex drive seems to be very high for the past 5-6 months as well. Higher than normal at least. I use to masturbate once or XXXXXXX twice a day, sometimes 3 times. But lately, it's been 4-6 times a day and occasionally more than that even. It seems very sudden and uncontrollable to me. This might also just be a lifestyle thing, because I have been at home for most of the last 5-6 months. When I am out, and working all day, it's not as if I feel the urge that much.

I have been experiencing some strange neurological issues for the past 1 1/2 - 2 years. It started one random day when I just woke up from sleep feeling really strange. My head felt very strange. I really do not know how to describe it, it just felt like something inside my head melted or something. I felt like I didn't really sleep at all, and I felt so tired, and felt like I could just sleep forever and never wake up.

After this episode, which happened again and again, I genuinely felt like I lost something inside my head. I felt different. I felt like I wasn't as intelligent as I use to be before, sometimes I felt like a vegetable, just incapable of doing any thinking whatsoever. My memory seemed to get weaker. Just my general cognitive performance seemed to have gone down drastically. This all might have been because I was sleep deprived, and still am. I do not remember when was the last time I had a good night of sleep, and woke up feeling fresh. I have had a very difficult relationship with sleep for the last few number of years. I was never really able to address this issue, and just continued to live with it.

Related to this, I have also had this strange phenomenon with my eyes, where every few minutes they would become unfocused. Like I would feel as if my pupils would dilate and my eyes would not be focused on anything, making my vision blurry. And my head would also feel very unfocused when this happens, like it was tired or something and was taking a break. Then I would force myself to focus and return to the present. I am not sure what is the cause for this either, but I have had this since the past 1 1/2 year at least. And the frequency at which it happens has also been fluctuating. It usually happens every few minutes.

So that's all I have. If any one of you can give any professional advice or explanation for my condition(s), I would really really appreciate it. If you can give me some direction, clarity, or suggest some questions that I can ask to my doctor, or suggest me to visit any specialist or get any specific tests done, that would also be helpful, because I have no clue what is going on with me.

Brief Answer:
The bladder size could have been like this right from the beginning

Detailed Answer:
Thanks for using the Ask a Doctor service.

I have gone carefully through the entire details. I do not think that your bladder has shrunken. Rather, possibly its size was just like that and has recently been detected after your problem relating to urinary frequency. This has partial relationship with anxiety which flows in a vicious cycle irrespective of the contributing cause. If I had a patient like you, I would have asked him to take Cap. Prodep (Fluoxetine) 20 mg once after breakfast for at least a couple of months. You can ask your doctor to write it down for you. I would insist you to walk regularly in the morning. I would also insist you to take some Vitamin D supplement (60,000 IU) once weekly for 6 weeks. I hope that with these you can see some improvement.

Thank you for your response, i have this query : How can my bladder size have been so small all along as
I have never had such issues with Frequency or urgency. I would drink water normally, as everyone else, and never had any issues. I have been having this just for the past 5-6 months. Before that I easily use to pass more than 250 mL , around 400-600 ish.

Brief Answer:
Bladder size can be increased

Detailed Answer:
I understand your query but do not have the answer to this one. Rather, it would make sense to act systematically to increase the bladder size. The following scientific publication can help:
