I understand your concern.
First of all, vitamin D deficiency is a common problem encountered these days. Vitamin D is a vitamin which can be synthesised in human body. The problem arises in poor people who cant have enough dietary source, in vegetarians because the main source is animal source, in old age because the overall formation, absorption, metabolism etc become reduced, in people not receiving enough sunlight and in people having dark complexion.
Hence vitamin D deficiency is common in old age.
You can get a blood test for vitamin D or vitamin D3 done and as per reports, medication like vitamin D supplementation can be started alongwith calcium.

COPD directly cannot cause it

Hello.
Vit D deficiency is not directly caused by COPD. But in patients suffering chronically from COPD, appetite reduces and hence dietary deficiency can lead to it.
COPD is a chronic disease but if managed properly with medications, restrictions and physiotherapy, gives very good result and prognosis is quite good. The person concerned has to stop smoking, do chest physiotherapy, take inhalers and other medications as advised by doctor. With these, a COPD affected person may live many years provided he/she does not have other co-morbidities. Life expectancy might be 70-80 years or more depending upon all the factors.

Plus he had a triple bypass 4 years ago

Because he had a triple bypass 4 years ago, the comorbidities are increased.
If he has COPD under control with medication and/or inhalers etc, then it can be thought that the prognosis is in the better side. But otherwise considering his age and histories you have provided, the prognosis is uncertain and might not be good.
