Is The HIV P 24 Test Done At 87th Day Conclusive?

Question: Hello,
I had virginal intercourse with a sex worker about 6 months ago. After that i got tested for the same at 6 weeks mark where my result showed not detected for an anitibody test. At 8 weeks i again got tested using the rapid test and my result was not detected. Around 11 weeks I got Hiv DNA PCR test which showed NOT DETECTED for HIV-1. Since I was not satisfied I again got tested at 12 weeks mark for HIV P24 and antibody test using CMIA method and the report read NEGATIVE. Again around the 87th day I got tested for HIV P24 and Antibody which was NEGATIVE. Should i get tested after 6 months or am I fine to go. Also I was using a knife in the kitchen where i got cut around 2.5 months back should I get tested for HIV for the same and am I at any kind of risk for the same ? Doctor please advice as I am really scared and cannot concentrate on anything. Also do any kind of medicines affect the test results ??

In an unprotected intercourse with a HIV positive person the HIV viruses gets entered into the body and divides in innumerous numbers(millions of viruses per ml).

After a period of 2-6 week there is mounting of antibodies against the viruses. These antibodies can take at the maximum of 8-10 weeks of the period to get expressed.

There fore the infection can be analyzed by two ways -

A): By HIV virus detection: done by RNA- PCR. It tells us the number of copies of virus present in per ml of blood. By the latest of methods the viruses can be detected within 2 weeks of period.

B): By HIV antibody Detection: As told earlier once the antibodies appear maximum of 8-10 weeks then these antibodies to HIV virus persist throughout life. The protocol of the antibody tests done for HIV is as follows: One HIV antibody test at 6-8 weeks and then a follow up final test at 3 months (84 days or 12 weeks).

Antibodies once formed to HIV can never disappear. So if a person has HIV infection, 3 months of period is the time when we say to be fully sure of whether HIV is present or not.

Now correlating all the facts and interpreting with your situation you can get that all your tests are don beyond the set limit of time which we can get as PCR done after 2 weeks and antibody test done after 3 months.

So no further tests are requires and you are totally free from HIV.

I wanted to know as to have you read my reports ? And are they normal ? And also i had an operation 6 months prior to exposure so would that affect my result under any circumstances ? I was cutting something in my friends kitchen where i got slight cut is that a risk for HIV ? Please tell me what to do ?

I have given all my statements on the basis of thorough interpretation of both your reports and then correlating these with your exposure time.

I already reassured you for your negative status and again reassure you that in all the above situations your status is negative.

Your resorts are totally correct and are done at right time. To tell you more elaborately:

Your vaginal intercourse was 6 months back. The reports are done in August and then September. The DNA PCR report was done in August2014. It is enough of time for the same to declare the exact status of a person.

A DNA PCR after 21 days is most accurate. In your case it was done aroung more than 21 days so giving you accurate result.

Further the report of September 2014 for antibody test makes everything clear.

Any operation done 6 months prior to the exposure would not at all affect the test results. It has nothing to do with the test results, so just totally relax about the same.

Cutting something in your friends kitchen with slight cut is also not at all of any risk for HIV. So this is also not at all of any risk for HIV.

I reassure you fully that you are totally clear and you do not have HIV.

I would like to tell u that I had used a condom while my exposure but I really got very scared. I will not do this act ever again and put my self at risk. Can u please again reassure me that I am completely fine. And are u sure that from a knife cut it is not possible to get HIV ??? Thank you so much
